Services: To coordinate end of life care for Bristol and the St Peters Hospice at Home services and work within the patient’s home by shift work or roving visits (if required), working with partners from other organisations

Job Summary/Main Purpose – Under the leadership of the team manager

  1. To provide holistic assessments to patients and their families and carers to plan for a package of care via the coordination centre and Hospice at Home,

(By coordinating the service and working in a “roving” capacity, as service needs allow)

  1. To coordinate care for the end of life coordination centre and the Hospice at Home Service,taking the lead for resourcing care as required.
  2. To visit patients homes, depending on complexity of care needs and to support HCA lone working staff
  3. To work occasional full or partial shifts in a patients home should the need arise
  4. To develop effective working relationships with all internal and external allied Health Care Professionals; providing care advice regarding end of life and palliative care in the home.Taking direction from the CNS team in working hours and the advice line/Dr on call out of hours.

Principal Duties and Responsibilities

Leadership and Management

1To plan, coordinate and rationalise complex packages of care for Bristol end of life care and the Hospice at Home Service with the support of the Hospice at Home Team Manager, by developing rota’s of care for families in need, and in conjunction with other service providers/partners as required.

2To work in close partnership with other staff within the coordination centre taking the lead on decisions as required.

3To develop effective working relationships with other members of the Hospice multi- disciplinary team.

4To develop effective working relationships with other, external, domiciliary care providers and external partners

5To be included in decisions regarding development or review of policies, standards and procedures pertaining to end of life coordination and Hospice at Home

6To communicate effectively and to agreed arrangements with external allied health care professionals to ensure the patient and family receive coordinated care of a high quality.

7To support the hospice at home management team as required, including coordination and leadership as required

8Under the guidance of the HAH Team Manager/partnership managers, take part in the recruitment of new HCA staff by contributing to the shortlisting, interviewing and selection, using agreed methods and based on objective assessments against agreed selection criteria

9Have one full office day per month to fulfil some supervisory roles

Clinical

1To assess, plan, implement and evaluate end of life/palliative care provision for patients in their own homes, liaising with St Peters Hospice specialist services (CNS, Advice Line) as required

2To provide roving support, as appropriate, to the Hospice at Home HCA team whilst they are working in a patients house, to assess patients for extended care and clinical needs & to offer supportive input to compliment the CNS working practices.

3To participate in the team out of hour’s rota in conjunction with partners and other St Peters Community Teams.

4To provide bereavement support for families and carers following the death of the patient

5To participate in effective multi-disciplinary working in the community.

6To adhere to your professional code of conduct.

7To attend clinical supervision as provided for under hospice policy

8Maintain and promote the maintenance of good clinical record keeping in a complex partnership system that affects St Peters Hospice and other systems

Quality

1To respond to all referrals effectively and within agreed standards, in conjunction with Palliative Care Home Support and St Peters Hospice Access Team.

2To contribute to established Quality Assurance Programmes including clinical audit, implementing recommended changes from feedback and audit, so ensuring evidence based, quality specialist palliative care is being delivered to patients, carers and their families.

3To work within SPH policy and procedures,ensuring that all hospice at home standards are met and adhered to.

4To regularly attend and participate in team and organisational meetings to improve communication, representation and service provision/development.

5To contribute to data collection that feeds contractual obligations, standards and KPI’s

Staff Development

1To be responsible for own continuing professional development

2To participate and contribute to personal Individual Performance Review, and deliver the same for the HCA staff

3Contribute to the training and learning needs of internal staff, SPH Ambassadors, and external placements as required by SPH.

4Contribute to policy development that affects the staff for which this role is responsible

General – The post holder will

  • Provide cover for the Team Manager as required and appropriate.
  • Have access to confidential data on patients and services within St Peters Hospice. Failure to maintain confidentiality will lead to disciplinary action, which could ultimately lead to dismissal.
  • Be required to participate in the assessment of risk and thereby contribute towards clinical and corporate governance agenda as appropriate.
  • Participate in annual staff performance and development review of own performance
  • Be expected to produce work of a high standard and to promote quality at all times.

They must use all equipment provided to undertake their role safely.
  • Refrain from smoking in any area of the Hospice premises not designated a smoking area, to include a patient’s home.
  • Conform to the professional standards set by their relevant professional body. Including ensuring registration is current and continuous professional development is practised.
  • Uphold the philosophy of the Hospice;this is based on an interdisciplinary approach and employees and volunteers are required to participate in this concept.

Roving role

  • The roving nurse can visit patients to:-
  • Review care needs
  • Visit patients receiving end of life care to offer support to the family and care staff. Furthermore to act as a conduit of communication with the CNS teams
  • Offer additional support to the family/carer
  • Support the HCA staff working in the home
  • Support to the PHCT at times of high workload (e.g. assist with specific medications and review need)
  • Strive to ensure that all care needs are in place before the end of their shift to avoid an out of hours crisis
  • Offer supportive visits to patients receiving respite care to support the CNS teams
  • The roving role can also be used to cover full or part shifts if required.
  • Visit new referrals in the community to establish that care requirements are planned for and to risk assess the environment
  • Visit direct referrals from the acute sector to liaise appropriately with the patient, family and carers.
  • To check that all the appropriate paperwork is in place, or to advise appropriately
  • Take responsibility for the coordination of the service in the absence of the Team Manager
