General Statement of Duties: Under direct supervision of the Clinical Nurse Manager, perform work of moderate difficulty administering care, treatment, and medication to residents of Lucy Corr Village Assisted Living Neighborhood; additionally, suggest, promote and lead new initiatives; provide basic household, nursing and personal care and assistance to residents; promote the welfare and dignity of all residents and act as an advocate in the delivery of care; foster real and positive relationship with residents and colleagues; promote the mission and values of Lucy Corr Village both internally and within the community at large; and perform other work as required.

Description of Duties (not all inclusive):

  • Work with the Assisted Living Staff to create a home like environment for the clients who have chosen Lucy Corr Village as their new home.
  • Foster positive attitudes and relationships, and take a leadership role among Lucy Corr Village staff, supporting and coaching new staff and communicating concerns to management.
  • Act as liaison between associates, residents and leadership, vigilantly creating effective lines of communication.
  • Coach current employees who may need additional clinical training and/or assistance with developing interpersonal skills and positive relationships with residents and colleagues.
  • Assist new residents in overcoming trauma associated with moving into an Assisted Living facility, and orient them to Lucy Corr Village.
  • Encourage open and effective communication with and among peers.
  • Engage residents in cheery and reminiscent conversation.
  • Maintain good working knowledge in Assisted Living Regulations and Standards as defined by The Department of Social Service.
  • Maintain the smooth running of the Assisted Living Center.
  • Provide direction to, and work directly with, the Assisted Living staff.
  • Inspects medication carts, behind med techs to ensure continuous compliance and to identify reeducation opportunities. Will also ensure that medications are being refilled in a timely manner to prevent unavailability of medications.
  • Communicate pertinent information to AL Clinical Nurse Manager, physician, responsible party, and/or administrator (on-call).
  • Provide information and conduct tours for prospective residents and families of Assisted Living.
  • Ensure work orders are completed in a timely manner for items needing maintenance work or repair.
  • Accurately administer residents’ medications.
  • Administer emergency care to residents
  • Meticulously maintain and audit residents’ medical records, residents’ charts and assist with medication reviews.
  • Accurately record doctors’ orders and order medications and supplies as needed.
  • Implement immediate procedures to effectively deal with aggressive or agitated residents as required.
  • Ensure that Face Sheets, UAI, ISP are updated and maintained accurately and efficiently.
  • Collects lab specimens as ordered by the physician.
  • Accurately monitor and record resident’s medical signs.
  • Audit Medication Administration and Controlled Substance Sheets, Refrigerator and Outdated Medication Logs, and other documentation, which is required to be completed by LPS/Med Techs, diplomatically communicate to others needed corrective actions and/or improvements, and appropriately alert Clinical Nurse Manager to the same.
  • Make scheduled rounds on all the residents making sure their needs are being met.
  • Order oxygen for residents weekly and supplies as needed.
  • Conduct clinical shift change meeting daily.
  • Perform weekly assess clients who are not regularly exposed for skin break down.
  • Monitor residents' nutritional status and intake and make needed recommendations to AL leadership.
  • Accurately records notes and prepares reports as required.
  • Encourage, create, lead and assist with planned and impromptu activities and programs.
  • Conduct Environmental Rounds, appropriately complete necessary documentation and follow-up requests, alert Clinical Nurse Manager and/or other specialty departments of corrective actions needed.
  • Notifies the Clinical Nurse Manager of Assisted Living on an on-going basis of any changes, family concerns, interdepartmental concerns, staff concern and any deviation from normal activity of resident function.
  • Provide effective in-service training as needed.
  • Perform other work as required.

Physical Requirements: The ability to: speak, read and write the English language; communicate effectively; use dexterity of fingers, and perform fine manipulation, gross grasp, pushing and pulling with both the right and left arms, hands and fingers, and repetitive motion with both right and left feet, or effectively compensate for the same;lift 0 – 5 pounds continuously, 6 – 20 pounds frequently, and 21 to over 100 pounds occasionally, with 51 to over 100 pounds being on a rare occasion, bend, stoop, and reach above shoulders frequently, squat, kneel, climb/balance, use repetitive arm motions, and crawl occasionally, sit, stand, and walk with rest throughout an eight hour day, drive occasionally in a safe and responsible fashion.

Required Knowledge, Skills and Abilities: Requires excellent working knowledge of the principles and practices of basic household, nursing, and personal care, and the ability to instruct on, and provide care in an exceptional manner. Must possess and be able to consistently demonstrate good interpersonal skills, the ability to communicate effectively both orally and in writing, effectively coach and lead co-workers and foster real and positive relationships with residents and colleagues; must possess and be able to demonstrate a positive attitude, excellent work ethics and high standards. Requires the skill and ability to suggest, promote, and lead new initiatives. Must have good knowledgeof the principles and practices of nursing, emergency transfer procedures; pharmacology as applied for preparing and distributing medications; maintaining records and preparing reports, training and supervising personnel.

Minimum Education and Experience: Core curriculum for a diploma from an accredited school of practical nursing, or an equivalent combination of training and experience, is required. Current Practical Nurse License by the State of Virginia.
