Salary: AP4 Points 27-30 - £23,697

Hours of work: 37.5 hours per week including some weekend and evening work.

Holidays: 25 days and 10 public holidays pro rata

Waverley Care is Scotland’s leading charity offering support to people living with and affected by Hepatitis C and/or HIV

Key Purpose

To deliver one to one assertive outreach support and to facilitate our self management life skills programme to people living with Hepatitis C, residing in NHS Greater Glasgow and Clyde. To deliver a Hepatitis C testing service to those most at risk of contracting Hepatitis C

As part of the Waverley Care Hepatitis C Support Coordination team, and in collaboration with community based Addiction Nurses, Hepatitis C Specialist Nurses in clinical settings and referring partners, the post holder will:

·  Deliver a one to one assertive outreach support to individuals living with Hepatitis C as they move through their care pathway, from point of diagnosis. The primary focus of this service will be to support individuals to remain engaged and to maintain good health whilst they wait for treatment, to support individuals in their day to day lives, to support and encourage individuals into assessment and treatment for their condition and to support individuals to move on in their lives free of Hepatitis C.

·  Deliver and facilitate the Waverley Care Self Management Life Skills Programme to individual living with Hepatitis C. The primary focus of this service is to enable individuals to take control of their lives and their condition leading to making positive changes and moving on. The Life Skills Programme allow participants to coach, support and motivate themselves and each other by reflecting on their lives, their achievements and strengths whilst working on particular issues, behaviours and areas that could benefit from change. The programme provides participants with factual information and practical everyday skills, including the use cognitive behaviour life coaching tools and solution focused techniques to help participants manage their physical, mental and emotional health.

·  Deliver and facilitate opportunities for one to one and group peer support.

·  Deliver a Hepatitis C Testing Service to those most at risk of contracting Hepatitis C. This service will focus on promoting the value of testing and knowing your Hepatitis C status, testing people who have been at risk, and supporting individuals who have tested antibody/PCR positive to assessment in acute settings.

Main duties and responsibilities:

1.  Service User Support

·  Manage key worker responsibilities for a caseload of service users.

·  Work with service users to create a personal plan, using the Waverley Care baseline assessment tool, with identified outcomes and carry out regular reviews of these plans.

·  Provide one-to-one emotional support to service users to identify and overcome barriers to improving their health and well being.

·  Provide one-to-one practical support to service users including supporting people to attend case discussions, review meetings, assessment and treatment appointments, other appointment and provide appropriate advocacy work.

·  Facilitate the Waverley Care Life Skills Programme and participate in the development and design of this programme to ensure that it is responsive to needs of individuals along the Hepatitis C Care Pathway.

·  Facilitate opportunities for one-to-one and group peer support.

2.  Hepatitis C Testing

·  Promote the value of testing and knowing your Hepatitis and/or Blood Borne Virus (BBV) status.

·  Deliver a Hepatitis C Dry Blood Spot Testing Service in accordance with the Waverley Care Dry Blood Spot Testing protocol.

·  Support individuals who test antibody/PCR positive to attend assessment appointments in clinical settings

3.  Networking and Partnership Work

·  Receive referrals from statutory and non statutory services as well as self referrals, and to record the source of referrals.

·  Liaise and network with internal Waverley Care staff and with external agencies across NHS Greater Glasgow and Clyde, particularly with community based addiction nurses and Hepatitis C specialist nurses in clinical settings.

·  Attend Local Hepatitis C Planning meetings as instructed by the Senior Manager Community Projects

·  Publicise all aspect of the service delivery to health and social care services and to other specialist BBV services and addiction services.

·  Attend and present at local and national conferences/seminars when required

4.  Monitoring and Evaluation

·  Routinely monitor and evaluate all aspects of the service delivery.

·  Record and keep up to date records of all client interventions on the Waverley Care Service User Database.

·  Provide required plans and reports to our funders.

5.  Involving Others

·  Ensure service user involvement in the effective design, delivery and review of the service.

·  Encourage opportunities for volunteering across all aspect of the service delivery

·  Recruit, train and support volunteers.

·  Support, supervise and assess the involvement of SVQ student placements in all aspects of the service delivery.

6.  General

·  Participate in team meetings, case management meetings, team training and service planning.

·  Work to and adhere to all Waverley Care policies and practice guideline.

·  Participate in supervision and annual planning.

·  Identify and undertake relevant personal training, including training in Motivational Interviewing.

·  Work evenings and weekends as the service demands.
