HOURS:Negotiable up to full time

Job summary:
This post provides an opportunity to work within a high quality primary care environment delivering innovative healthcare to a marginalised population.
Job responsibilities
Undertake a range of general practice consultations as appropriate to meet the needs of the homeless population
Participate in GP rota undertaking hospital ward rounds as part of the mpath service, working alongside non clinical staff developing discharge and primary care plans for homeless in patients
Participate in the care of people suffering with substance misuse treated under the shared care model at the practice
Offer supervision and support to clinical and non-clinical members of the homeless service
Contribute to the development and delivery of an Inclusion Health training programme
Shared responsibility for maintaining excellent clinical governance and evidence based practice for the homeless health service
Person Specification
Essential / DesirableQualifications/training /
- Medical practitioner whose name is included on GP register
- Currently on a CCG performer’s list and not suspended from that list, or from the medical register
- DBS clearance
- Have had an annual NHS appraisal
- Evidence of further post graduate educational activities in relevant fields
- RGCP Diploma in substance misuse certificate level 1 or 2
Knowledge and Experience /
- Recent experience of working in general practice
- An interest in working with people with complex needs, including substance misuse and mental health
- Knowledge of the inclusion health agenda
- A willingness to acquire relevant specialist skills and knowledge required to meet the needs of the homeless population including RGCP Diploma in treatment of substance misuse
- Evidence of independent working in general practice
- Experience of working in an inclusion health environment e.g. working with homeless population or people with substance misuse issues
- Teaching experience
- Experience of supporting service change

- Excellent verbal and written communication skills
- Ability to adjust communication skills to meet the needs of the recipient
- Good listening skills in order to understand the needs of others
- Fully competent with clinical IT systems
- Excellent time management
- Strong leadership skills
- Excellent negotiation skills and the ability to resolve conflict
- Working knowledge of EMIS IT system
