Job Summary/Main Purpose

  1. To provide leadership, co-ordination and development of the Day Hospice and associated Services. (Management/Clinical split: 30%/70%)
  2. To provide clinical leadership and support to team members ensuring appropriate assessment and provision of management plans for new patient referrals.
  3. To contribute to Quality Assurance Programmes, implementing and evaluating recommended changes, so ensuring research-based, quality specialist palliative care is being delivered to patients, carers and their families.
  4. To work in partnership with Primary Health Care Teams acting as a resource to provide specialist clinical advice, support and education ensuring practice is evidence based.
  5. To be responsible for a shared caseload and liaison with other members of the hospice inter-disciplinary team.
  6. To act as a resource to other professionals, both internally and externally, on specialist palliative care issues.
  7. To contribute to strategic and operational business planning.

Principal Duties and Responsibilities

Management

1To coordinate Day Services on a day to day operational basis and participate in the management of both paid and unpaid staff.

2Be responsible for allocating workload to Day Services staff in close liaison with other members of the interdisciplinary team; ensuring organisational standards are met.

3Provide the management of the Day Services Team who provide specialist palliative care to patients and their families in the Day Hospice and any appropriate venues, under the guidance of the Clinical Services Manager and Director of Patient Care.

4Manage the Complementary Therapy Service delivering therapies to patients in Day Hospice, IPU, Outpatients and Outreach venues, through the Complementary Therapy Coordinators.

5In collaboration with the Clinical Services Manager participate in the development and implementation of both strategic and operational service plans and budgets.

6Provide representation as required on professional committees within SPH and externally, offering expertise and disseminating information to the Day Services Team.

7Develop effective working relationships with other voluntary sector, private and public sector organisations.

8Coordinate with other departments in order to develop new services against strategy and patient need.

Clinical

9To supervise the assessment, planning, implementation and evaluation of specialist palliative care provision for patients and their families within Day Services, ensuring external allied health care professionals are informed and communicated with effectively to ensure the patient and family receive coordinated quality care.

10Ensure all referrals are dealt with effectively and within agreed standards.

11Provide supervision and support for staff who daily manage complex psychological needs of patients and families, to prevent accumulative work related stress.

12Contribute to the training and learning needs of internal staff, SPH Ambassadors, and external placements as required by SPH.

13Maintain computer records on patients and personnel records of staff as required in accordance with Information Governance.

Quality

14Actively participate in Hospice policy development and implementation, proposing changes either individually or in collaboration with others.

15Actively participate in audit to monitor quality of service, and actively develop service to ensure standards are met, and in doing so contribute to the achievement of CQC Standards.

16Act as team Health and Safety Representative; responsible for Risk Assessment, Workplace Hazard assessment, COSSH assessment, and ensuring staff attend all required Statutory Training.

17Take responsibility for identifying areas of practice that need improving and ensure thatstaff deliver high standards of care within the directorate and in compliance with their own codes of professional conduct.

Staff development, Research and Education

18Support Day Services staff with their professional development, via Individual Performance Reviews, support of further study, and provision of managerial supervision as appropriate.

19Assist in the recruitment, selection and development of staff.

General – The post holder will

  • Provide cover for colleagues as required and appropriate
  • Be a duty manager for the hospice on an agreed rota, if required
  • Have access to confidential data on staff, patients and services within St Peters Hospice. Uphold the hospice policy on confidentiality in particular data held about staff, patients and services within the organisation; be aware of consequences of failure to comply.
  • Be required to participate in the assessment of risk and thereby contribute towards clinical and corporate governance agenda as appropriate.
  • Participate in annual staff performance and development review of own performance.
  • Be expected to produce work of a high standard and to promote quality at all times.

They must use all equipment provided to undertake their role safely.
  • Refrain from smoking in any area of the Hospice premises not designated a smoking area.
  • Conform to the professional standards set by their relevant professional body. Including ensuring registration is current and continuous professional development is practised.
  • Uphold the philosophy of the Hospice; this is based on an interdisciplinary approach and employees and volunteers are required to participate in this concept.
