Details of Service / The General Practitioner(s) will ensure that provision of service under the agreement with the Health Service Executive South is carried out to a satisfactory standard and complies with Health Services Executive policies, procedures and standards.
The General Practitioner(s) shall not sign, transfer or sub contract, the agreement or any portion thereof without the prior consent in writing of the Health Services Executive South.

Purpose of the Post / The purpose of the job is to provide quality general practitioner medical services to clients who present to our services.
Principal Duties and Responsibilities / (A) Homeless Services
The General Practitioner(s) for Homeless Services for initial period to 22 June 2015 will be required to:
(i)  As part of a multi-disciplinary team, provide treatment to clients who present or are referred to HSE clinics and who are regarded as Adult Homeless Persons. Locations, times, dates and duration of clinic to be agreed periodically between the General Practitioner, and Project Manager, Adult Homeless Services or an appropriate nominee of the Health Service Executive.
(ii)  Co-operate with other health care professionals engaged in services for the Homeless such as Public Health Nursing, Addiction Services, Mental Health Services, Hospitals, General Practitioners, Public Health Doctors, etc.
(iii)  Medically assess Adult Homeless Persons presenting and treat appropriately
(iv)  Screen clients for relevant viral diseases, evaluate the results, treat and refer to specialist services where appropriate.
(v)  Provide appropriate vaccination programs to clients under your care.
(vi)  Provide primary medical care in emergency situations to clients who do not yet have medical card cover.
(vii)  Attend clinical team meetings where clients’ cases will be discussed and treatment plans developed with the care teams.
(viii)  Participate in appropriate meetings of General Practitioners working in the Primary Care Service and when appropriate offer views on policy, operational issues and service developments.
(ix)  Participate in continuing medical education to update skills in treating the Adult Homeless Population
(x)  Participate in relevant research appropriate to your principal duties and responsibilities.
(xi)  Liaise with relevant community groups where appropriate.
(xii)  Maintain all appropriate records (including on Safetynet ICT system) in relation to clients in accordance with best practice
(xiii)  Co-operate with making the clinical records available to potential registered doctors and other health care professionals where appropriate
(xiv)  Work for 30 hours per week at locations based in Cork City, currently these include,
a.  Simon Hostel, Anderson’s Quay
b.  St. Vincent de Paul Hostel, Anglesea Terrace
c.  Administrative Sessions – Lough Community Centre, Greenmount
Session duration and locations to be agreed as per service demand.
(xv)  Co-operate with other health care professionals engaged in services for the Adult Homeless and will assist and support all other members of the Cork City Multi-disciplinary Team.
(xvi)  Co-operate and work with Probation, Prison Services Community Welfare Services and Homeless Service Providers.
(xvii)  Maintain contacts with the Universities and Training Colleges for Medical Nursing and Social Care Students and Trainee Counsellors.
(xviii)  Assist in evaluating the service.
(xix)  Provide the Project Manager, Adult Homeless Service or any other appropriate person in charge (Operations Manager, General Manager) appropriate reports when requested to do so.

(B) Addiction Services
The General Practitioner(s) for Addiction Services for initial period to 22 June 2015 will be required to:
(i)  Cover clinic sessions in relation to HSE South Cork and Kerry
(ii)  Cover G.P. Methadone Programme services (including initiation of treatment; stabilisation and detox; transfer to community level I G.P. of stable clients as appropriate and clinically indicated and in line with ICGP guidelines) on a sessional basis in Drug and Alcohol Services.
(iii)  Work closely with the Director of Arbour House and the multi-disciplinary teams in relation to client care-plans.
(iv)  In relation to any operational/administrative issues arising to link in with the Regional Co-ordinator for Drug and Alcohol Services.
(v)  Link in as necessary with the GP Co-ordinator
(vi)  Work within ICGP Guidelines and relevant HSE policies and procedures.
(vii)  Be available by phone contact outside of these sessions in relation to any significant client issues that should arise. To attend multi-disciplinary team meetings.
(viii)  Ensure that all patients treated by him/her are properly registered on the Central Treatment List.
(ix)  Ensure all treatment provided (dosage, detoxification supervision), dispensing arrangements, frequency of visits, and urinalysis will be guided by best practice guidelines as defined by the Irish College of General Practitioners (ICGP) Best Practice Committee for Methadone Prescribing.
(x)  Participate in regular audit as required by the ICGP Joint Audit Review Group or such other review group as may be agreed HSE South and to follow any recommendations regarding practice made by such review group.
(xi)  Ensure no fees (except those paid by the HSE South) will be charged to patients for methadone treatment.
(xii)  Participate in Continuing Medical Education activities related to the field of addiction treatment to a level acceptable to HSE South
(xiii)  In the best clinical interest of the patient, to endeavour to transfer stabilised patients to a Level 1 G.P.
(xiv)  Complete and return to the Health Research Board National Drug Treatment Reporting System (NDTRS) forms within a timeframe as set down by the Health Research Board and any other reports deemed necessary

Eligibility Criteria
Qualifications and/ or experience / 1.  Be a medical practitioner who is registered otherwise than provisionally or temporarily in the Register of Medical Practitioners in Ireland or be entitled to be so registered by virtue of his/her registration in any other register of medical practitioners.
2.  The General Practitioner(s) must be qualified as Level 1 General Practitioner for care of patients under the Methadone Protocol Scheme. In the event that for whatever reason the General Practitioner ceases to be registered, the contract will as a consequence cease to have effect.
Skills, competencies, qualifications and/or knowledge / ·  Demonstrate evidence of ability to empathise with and treat clients, relatives and colleagues with dignity and respect
·  Display evidence-based clinical knowledge in making decisions regarding client care
·  Demonstrate effective communication skills including: the ability to present information in a clear and concise manner
·  Demonstrate understanding and/ or experience of the Irish health services and strategic initiatives designed to develop the health services here
·  Demonstrate ability to manage deadlines and effectively handle multiple tasks
·  Demonstrate awareness and appreciation of the service user
·  Demonstrate the ability to work with multi-disciplinary team members
·  Demonstrate a working knowledge of General medical Services scheme
·  Demonstrate an adequate working knowledge with computers (I.T. skills). Be able to produce reports/statistics
·  Demonstrate awareness/knowledge of government strategies on homelessness
·  Demonstrate awareness/knowledge of government strategies on Addictions
·  Demonstrate awareness of issues/health problems particular to the cohort of people
·  Demonstrate understanding and/ or experience of the other services/disciplines which may need to be engaged with to treat the service user

Remuneration / The Rate of Remuneration will be €71.50 per hour.
Hours of Attendance / Homeless Services
Hours of Attendance will be agreed between the General Practitioner and the Project Manager, Adult Homeless Services or other appropriate person in charge for the purpose of this contract. The contract period for initial period to 22 June 2015 is for 30 hours per week subject to review on an annual basis or whenever the service needs demand.
Addiction Services
Hours of Attendance will be agreed between the General Practitioner and Co-ordinator Drug & Alcohol Services or other appropriate person in charge for the purpose of this contract. The contract period for initial period to 22 June 2015 is for 38 hours per week subject to review on an annual basis or whenever the service needs demand.
Annual Leave / 8 hours per 100 hours worked
Superannuation / Sessional employees are not part of the Superannuation Scheme.
Age / Age restrictions shall only apply to a candidate where he / she is not classified as a new entrant. The candidate who is not classified as a new entrant must be under 65 years of age on the 1st day of the month in which the latest date for receiving completed application forms for the office occurs.
Health / A candidate for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.
