Registered NurseHospice Care

Part-time Evenings 4p – 9p

Qualifications

A HospiceNurse is currently licensed as a registered nurse in the state of Massachusetts with at least two years of experience in medical-surgical, oncology or community health nursing care required.

Functions

1.Assumes on-call coverage during evening hours and responds to patient/familyneeds per Hospice of CNA policies and procedures.

2.Able to assess and respond to the needs of patients and families in varied settings.

3.Accepts patient/family assignments and makes home visits as requested.

4.Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.

5.Able to make meaningful observations and write comprehensive accurate reports.

6.Available for weekend and holiday duty as scheduled per Hospice of CNA policies.

7.Assumes responsibility for responding to patient/family needs.

8. Communicates with attending physicians, other Hospice staff members and other agencies as needed to coordinate optimal care and use of resources for patient/family.

9.Maintain regular communication with the Hospice Coordinator, and/or other Hospice team members concerning patient/family care needs.

10.Maintain regular communication with the attending physician concerning patient/family care needs.

11.Obtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan

of care.

12.Maintain up-to-date patient records so that problems, plans, actions and goalsaccurately and clearly stated and changes are reflected as they occur.

13.Accept responsibility for coordinating physical care of the patient by teachingprimary caregivers, volunteers, employed caregivers, or by providing direct careas appropriate.

14.Supervise and document the HHA plan of care every 2 weeks.

15.Informs the Director of Hospice or designee of unusual or potentially problematic patient/family issues.

16.Provide appropriate support at time of death and period of bereavement.

17.Participate in Hospice of CNA’s orientation and in-service training programsfor professional staff.

18.Participate in agency and community programs as requested to promote professional growth and understanding of Hospice care.

19.Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which aid in determining policies.

20.Accept other assignments as appropriate.

21.Supports the philosophy of the agency.

22.Adheres to personnel policies.

23.Contributes to the process of Agency Performance Improvement.
