Job Description – Family Support Worker
Reports to:
Headteacherand Senior Management Team
Main Purpose of the Job:
To work alongside the schools education team, care team and other professionals within the school to support pupils and their families who are identified as requiring additional interventions (Tier 3 and4 Hampshire Safeguarding Thresholds).
To manage a case load of families and support a multi professional intervention in partnership with other agencies.
To support individual children and their families to address issues relating to health, welfare and social care to enable them to engage fully and maximise opportunities for personal development and educational achievement.
Specific Accountabilities
Main Activities:
To maintain good working relationships with all agencies and individuals involved in the delivery of services and the early identification of need.
To establish effective and empowering relationships with parents/carers.
To adhere to appropriate confidentiality and information sharing protocols and legislation with regards to sensitive data between families, staff and other relevant agencies.
To provide information and signpost parents/carers as appropriate.
To develop and deliver a preventative service and provide support to improve the quality of care both within and outside the family environment.
To work alongside the school’s other Family Support Worker, the Care Team and other professionals in the area to deliver a seamless service to families.
To facilitateparent groups once qualified to do so.
To take part in service evaluation, monitoring and parent consultations.
To take an active role in the support of pupils in multi-agency meetings.
Support to Families:
To work in a non-discriminatory, culturally sensitive manner, encouraging all families to take an active and positive role in their children’s education.
To undertake outreach work, with parents and carers in the home to provide information, guidance and support on a range of parenting issues.
To provide positive experiences for children which enable them to achieve appropriate developmental and academic milestones and ensure that the needs of each individual child are catered for.
To maintain detailed recording and assessments of each family/child’s progress within their specific care plan and ensure accuracy of recorded information and data.
To carry out planned direct work with children and families in the community and group work as required.
To work to deadlines and within specific timescales for monitoring, recording and other such purposes.
To participate fully in any Individual Education or Behaviour Plan (IEP/IBP) Child Protection, Child In Need or Early Help procedures involving children supported by the school.
To form a bridge between supports delivered in school, parents and carers to enable a consistent approach to supporting children’s individual needs.
To deliver parenting programmes to meet the needs of identified groups of parents and individuals who are unable to access the groups.
Other requirements:
To undertake a review of his/her practice and professional development to ensure that necessary skills, knowledge and understanding are kept updated.
To attend appropriate conferences/seminars/training events with regard to current issues within School.
To contribute to the overall ethos/work/aims of the School.
Be aware of and comply with policies and procedures relating to child protection, health and safety, security, lone working, equal opportunities, confidentiality and data protection.
To undertake other duties as agreed with the Senior Management Team commensurate to the grade and responsibilities of the post.
To produce a termly impact report for presentation to Governors.
Special Conditions: Hours and days are set and working hours are 30 hours per week, Term time only. Working hours are 8.55am – 4.00pm Mon and Tues, 8:55-3:10 Wed-Fri, but some flexibility in hours will be required.
Current Driving Licence and access to a car is also required.
Salary: Grade D £19,846 – £22,338 pro-rata (actual £14,118 - £15,889)
