Job Summary:

The Director, Partnerships to Improve Community Health is an accomplished community health leader who advances EMHS community health strategy: To promote a culture of community stewardship; to partner with key community, healthcare and public health stakeholders to address high priority health issues; and to encourage vibrant communities (policies, systems and environments) that lead to improved population health outcomes in Maine.

The Director manages the delivery of grant funded community prevention services. These include a $4M/3 year CDC funded Partnerships to Improve Community Health Initiative (one year remaining), and the start-up of Maine DHHS sub-award contracts for delivery of Tobacco and Substance Use Prevention Services in Maine's Public Health Districts.

The Director engages with EMHS member organizations and community partners to foster innovative, collaborative initiatives that improve health outcomes. S/He secures and leverages external funding in partnership with community and public health stakeholders to achieve positive community impact. The Director must possess strong collaborative leadership, outstanding interpersonal communication and advanced project leadership skills. S/He should have strong program and financial management skills along with program design, implementation and evaluation experience.

The Director reports to the Chief Community Health and Grants Officer, and assures the successful implementation and evaluation of current programmatic initiatives, while nurturing partnerships, networks and community collaborations that achieve a collective response to preventing disease and promoting community health throughout Maine.

S/He oversees the implementation of project activities, supervises staff, coordinates and monitors the performance of contract partners, develops materials, provides training and presentations, facilitates meetings and project communications and ensures that all required reports and documentation are submitted accurately and on time.

The Director works closely with the Chief Community Health & Grants Officer and Director of Grant Services to identify and pursue external grant funding opportunities to advance priority community health improvement efforts. The Director nurtures and maintains relationships with foundations and governmental funding agencies to leverage strategic philanthropic and governmental investments which result in improved community health outcomes. The ideal candidate will have a successful track record attracting and investing external resources for community health improvement

This role is classed as a temporary, full-time, long-term (grant funded) position.

Job Functions and Duties
Leadership
· Serve as an EMHS ambassador, identify partnership opportunities, and build collaborative relationships with public health officials, healthcare partners, community organizations, and other stakeholders.
· Serve on relevant strategic committees and task forces to promote and support system-wide community, population, and public health initiatives.
· Provide leadership, direction and management of funded collaborative initiatives.
·  Provide formative and developmental leadership to emerging networks and community coalitions aligned to address priority health issues.
·  Work with partners to develop community and public health capacity to assure the conditions for community health improvement.
·  Recruit and develop competent and skilled professional staff to engage in collaborative health improvement initiatives.
Advancement
· Develop strategic program plans to leverage partnerships for prevention and population health improvement.
· Explore and engage with funders and partners to develop shared investment models.
· Actively seek and secure grants, cooperative agreements and philanthropic support for community health improvement.
· Oversee communication and dissemination efforts to effectively reach key audiences and stakeholders, sharing models and opportunities for collective impact, as well as program results and lessons learned.
· Facilitate performance acceleration and improvement strategies with projects and partners.
Administrative & Program Oversight
· Direct program staff and ensure all reports and documentation are submitted to funders in a timely fashion.
· Convene meetings and conference calls with funders, partners, project officers and evaluators and key staff.
· Assure the systematic capture of useful data to inform evaluators, partners and stakeholders for continuous program and performance improvement.
· Establish and oversee program and contract management strategies, systems and mechanisms.
· Develop and manage work plan timelines to ensure successful completion of program deliverables.
· Coordinate the convening of partner teams to advance strategic work plan efforts.
· Work with community network and coalition leaders and their partners to engage in population-based strategies to implement policy, systems and environmental enhancements.
· Oversee the implementation of all committed project work plan activities.
· Oversee the coordination and monitoring of sub-award partners to achieve performance milestones.
·  Provide information, support and technical assistance to intervention partners to facilitate collective achievement of target outcomes.
·  Work with partners and funders to support peer learning and regional scaling and dissemination of best practices and evidence-based interventions.

Education and Experience

A Bachelor's degree is required; a Master's degree is preferred. Ten years of progressively responsible experience with community health program management including 5 years senior program leadership and people management. Experience implementing state and federally-funded programs and management of sub-awards preferred.

·  Ability to establish and maintain strong cooperative relationships with internal and external staff, partners, representatives of governmental and non-governmental agencies and the general public

·  Ability to define problems, collect data, establish facts, and draw valid conclusions

·  Ability to think analytically and apply sound judgment, solve problems, make effective decisions

·  Ability to envision new opportunities, and to align people and partners to achieve shared outcomes

·  Strong interpersonal & communication skills, including written, oral, and computer skills

·  Strong software and computer skills, including Microsoft Office (including Word, Power Point, Access, and Excel), database and website utilization and/or development as needed

·  Experience supervising, motivating and developing staff

·  Ability to travel throughout Maine
