Rota & Responsibilities / Week plan:
Ward: Daily registrar ward round. Consultant ward round approximately twice weekly although will review new and unwell patients daily.
Review of new admissions including confirming fitness for chemotherapy. Organising specialist tests and discharge summaries.
Ward chemotherapy prescribed weekly on Wednesday afternoon in conjunction with pharmacy team.
Twice weekly formal ward rounds (Tuesdays 14.30-14.30 and Fridays 08.30-09.45) with extended multidisciplinary team - patients presented by ward registrar
Excellent exposure to new oncology diagnosis and management, admissions for high dose chemotherapy, stem cell and cord transplants and symptom control.
Day care: Daily morning theatre list run by senior staff grade enabling experience to be gained in bone marrow aspiration and trephines, lumbar puncture and intrathecal chemotherapy.
Review of daycare admissions including attendances for routine chemotherapy and ensuring fitness for above procedures and general anaesthetics. Clinical review and further management of unwell patients admitted from outpatient’s clinic or via shared care hospitals.

On call: Nights and long days you cover both McElwain Paediatric Ward and the TYA Ward.
ST3 cover the on call as middle grade. During on calls will carry the shared care phone, the initial point of contact for both parents and shared care centres. Responsibility for advice on new oncology referrals and management of existing patients admitted to local services i.e. management of febrile neutropenia / chickenpox exposure / symptom management. Well supported by consultant on call.
Responsibility for day care chemotherapy prescribing on night shifts.

Drug development consultants and team enabling exposure with phase 1 and 2 clinical trials.
Cohesive friendly department with great emphasis on education and maintaining excellent clinical standards in patient care. International patients are regularly managed.
Training opportunities / Designated Clinic Week fixed into the rota. Opportunity to attend wide range of consultant clinics including leukaemia/lymphoma, solid tumours, neuro-oncology, radiotherapy, bone marrow transplant, late effects and endocrine clinics. Time available to complete audits, attend radiotherapy planning meetings and shadow palliative care team on home/hospice visits.
Teaching / Weekly Thursday morning consultant led teaching 8.30-9.30am. Covers wide range of oncology topics including individual conditions, symptom control, radiotherapy, chemotherapy side effects and specific oncology emergencies i.e tumour lysis syndrome
Weekly Tuesday afternoon teaching programme 1.30-2.30pm: Mix of consultant and trainee presentations including audit presentations
Weekly Leukaemia and Lymphoma Morphology Meeting: Review of diagnostic and prognostic bone marrow aspirations by haematology consultants
Weekly Solid Tumour multidisciplinary meeting (Tumour Board) – review of new and post treatment solid tumours including pathology, radiology and surgical assessments
Weekly Neurooncology multidisciplinary team meeting- Review of new and post treatment brain tumours
Regular teaching on consultant led ward rounds
Regular morbidity and mortality meetings
Regular lunchtime and late afternoon lectures on oncology topics at the Institute of Cancer Research (ICR) situated next to the Royal Marsden site.
Top tips / Try and do an audit, project or guideline. All of the consultants are involved in lots of interesting research and happy to help.
The Royal Marsden Hospital run Good Clinical Practice (GCP) courses. Excellent for all trainees, not just those involved in research.
Read the Paediatric Oncology Shared Care Guidelines, available in all North and South Thames shared care units. Key areas would be Febrile Neutropenia, Tumour lysis Syndrome and Symptom control.
Go through one patient’s Bone Marrow/Stem Cell Transplant protocol in the first week. Each protocol is individualised with relevant investigations, medications and drug /virology monitoring.
Try and review some of the morphology slides in the haematology laboratory with the haematology registrars / consultants.
