AGENDA Membership Meeting December 7, 2017 Cooper Green Health Services

  3. Special Speakers: Cooper Green Hospital Services
  4. Armika Berkley, Director of Cooper Green Mercy Health Services.
  5. Cooper Green is open.
  6. The only thing that Cooper Green has stopped providing is incare Services.
  7. Cooper Green provides Urgent Care, Primary Care and 19 Care specialist.
  8. Urgent Care center is open to negotiating with your organization to provide urgent care services at a reduced co-pay.
  9. Urgent Care is open 7 days a week.
  10. Enroll two ways: call or enroll in person at the enroll desk in the lobby or urgent care (open 5 days a week in lobby).
  11. Income Eligibility-is determined by household income. (Proof of dependents and income is now required.)
  12. Co-payment scale is $0, $10, $20.
  13. Arrangements can be made for patients who are deaf.

  15. Michelle wants to reiterate the good customer service at Cooper Green.

  17. Jeff will start to give reports from the board meeting in order to ensure communication between members and the board.

  19. Point in Time- January 23-24, 2018
  20. Gordon wants to expand the geographical areas where we complete PIT. This will take a collaborative effort with One Roof members.
  21. Springville Police Department maybe able to assist with St. Clair County.
  22. Tarrant has a lot of train lines that would have a population we could survey.
  23. Michelle wants to ensure safety so One Roof will be using consumers or former consumers from the street.
  24. If you know any current or former people experiencing homeless who would be good team leaders, provide One Roof with contact information. We will give a gift card for any participants.
  25. Fairfield could possibly be an area we could survey.
  26. Avondale Park will be one of the areas we will survey.
  27. Parrish from YWCA could connect us with court advocates and Pell City shelter coordinator.

  29. If you know someone who will need transportation to the warming stations, contact One Roof Outreach team.
  30. The appropriate referral for Outreach Team is someone who is homeless.
  31. Tionne, Aubrey, Faith and Greer are our Street Outreach team.
  32. You can give clients the Outreach Team number.

  34. Next Membership Meeting-January 4, 2018, 2 p.m. in 2nd floor cafeteria of Cooper Green Health Services. Topic: Point in Time & Project Homeless Connect. This will be the first planning meeting for ALL Project Homeless Connect Service Providers.
  35. Project Homeless Connect (PHC) will take place on February 24, 2018.
  36. Monthly Legal Help Desk: January 9, 2018 8:30 am-12:00 pm, located in the One Roof Office, 5th floor Cooper Green Mercy Health Services.
  37. One Roof/ Coordinated Assessment Holiday Hours: Closed December 25-26, 2017 & January 1-2, 2018.

  41. Foundry has opened up a new location in Pelham. The open house will be December 14-16.
