Completion of this form is required for all Students, Officers of Administration, and Support Staff who conduct ‘Columbia Travel’ to medium, high, or extreme travel risk destinations, as defined by International SOS (ISOS) Global Risk Ratings. Please note that a location’s risk rating is defined at the city, region, and/or country level, and the highest rating associated with your destination must be used for the purposes of this form. It is the traveler’s responsibility to ensure that they have complied with all requirements mandated of their respective school/unit and by theInternational Travel Planning Policy.

For those conducting group travel where all travelers will be participating under the same itinerary, please designate a single individual to submit this form on behalf of the group.
