This application applies to all goods and services vendors, including but not limited to security, amusement rides/devices, stage, fencing and seating set-up, animal rides, rock wall/bungy and other labor.

Payment shall be made for your services 1) by credit card on the Friday before the Parade, or 2) by check on the Monday following the Parade.
If contract pertains to a percentage of sales, then the ______percent shall be due and payable at 6 p.m. on the day of the event.
INSURANCE REQUIREMENTS:
1. ACORD Certificate of Insurance
Applicant will provide an ACORD certificate of insurance reflecting policies that are in effect for the duration of the festival for each of the following coverage types:
a) Commercial General Liability Insurance with limits of at least One Million Dollars ($1,000,000) per occurrence, subject to an annual aggregate of at least Two Mil1ion Dollars ($2,000,000);
b) Automobile Liability Insurance with a limit of at least One Million Dollars ($1,000,000) per occurrence;
2. Required Endorsements
Applicant’s insurance shall include the following:
a) A separate endorsement naming the "The Irish Congress of Southern California” and the “City of San Diego and it’s officers, employees, and agents” as additionally insured under the Commercial General Liability and Automobile Liability policies; Address for the Irish Congress is 415 Laurel Street PMB 113, San Diego, CA 92101 and address for the City of San Diego is 1200 Third Street, No. 1000, San Diego, CA 92101
b) A separate endorsement providing that coverage is primary and non contributory under the Commercial General Liability and Automobile Liability policies;
c) A separate endorsement with waiver of subrogation
d) Commercial General Liability and Automobile policies must provide that the insurer will waive all rights of subrogation.
NOTE: If an applicant does not currently own any vehicles, and has obtained automobile insurance coverage for "hired autos" and "non-owned autos," the applicant may submit a Declaration to this effect.

All of the above are REQUIRED. Your participation will not be accepted without their submittal. You must provide all of these documents with your signed contract.
RELEASE OF LIABILITY: IMPORTANT You must read and sign this release of liability for the application to be valid:
I/We hereby release (1) the Irish Congress of Southern California, Inc., its Board of Directors, members, volunteers and representatives; (2) The City of San Diego, and its respective elected officials, employees, agents and representatives; and (3) agents and employees of Balboa Park from any and all liabilities whatsoever for personal injury, property damage or loss, specifically including, without limitation, injury or damage to booth contents, merchandise, employees, contractors, guests, customers, invitees, and any other third party, resulting in participation at this event. I/We further agree to abide by the Rules and Regulations governing participation in this event as explained in the information accompanying this application.
