/ CARE COORDINATION FOR CHILDREN POLICY MANUAL

I. PURPOSE STATEMENT

The CC4C program (Care Coordination for Children) will perform assessments/screenings using the SWYC (Survey of the Well Being of Young Children) standardized tool to improve communication and identification of developmental and/or social emotional issues with parents and patients to improve health outcomes.

The CC4C completes the age appropriate SWYC screening tool based on the pathway.

IV. PROCEDURE

  1. The CC4C/parent will work to complete the age-appropriate SWYC screening tool within first 30 days of initial contact with family.
  1. The CC4C will score the completed form per appropriate guidelines.
  2. Based on the results, the CC4C will determine if follow-up and referral is needed.
  3. Results within normal range are communicated to parents and PCP and no further follow-up is needed. The CC4C will provide parent education materials.
  4. If screening results indicate the need for further assessment, the results will be share with parents and PCP.
  1. The CC4C will provide parents with appropriate follow-up information and resources based on results.
  2. The CC4C will provide PCP with abnormal screening results and follow-up plan.
  3. Abnormal SWYC screening results will be repeated every 2 months for children older than 18 months enrolled in the CC4C program unless they are currently receiving services to address the findings/results. This information should be documented in the CHA.
  4. As part of the treatment plan for the child, the abnormal SWYC screening results will be shared with providers involved in child’s treatment plan.
  5. The SWYC screening tool will be scanned and uploaded into the child’s chart in CMIS and labeled accordingly.
  1. The CC4C will complete the age-appropriate SWYC screening with children placed in heavy/medium case status based on the periodicity schedule.
  2. The CC4C will complete the age-appropriate SWYC screening with children placed in light case status at the next scheduled contact and based on the periodicity schedule.
