LifeLong Links

The AAA’s, serving as the Coordination Centers for LifeLong Links (Iowa’s Aging and Disability Resource Centers) have partnered with hundreds of agencies and networks across the state to streamline Iowans’ ability to get information, referrals, and assistance for all of their service needs. Last year LifeLong Links:

·  Received 55,951 calls for support

·  Conducted 63,299 contacts with consumers (face-to-face, e-mail, etc.)

·  Assisted 17,890 people to find and receive needed services

·  Most common referrals made to: Insurance Counseling, Financial Assistance, Home and Community Based Services, Nutrition, and Housing Options

·  Continue to develop our statewide database of 10,000+ services across Iowa.

Case Management

The Area Agencies on Aging provide Case Management services to allow Iowans with the option of staying in the home of their choice while reducing the Medicaid costs to the state of Iowa. Last year your AAA’s provided these services to almost 10,000 Iowans which saved over $530,000,000! This keeps more money in people’s pockets and reduces the strain on Medicaid.

Nutritious Meals

The Area Agencies on Aging, working with local partners provided 2,597,350 meals to Iowans. Of these meals, 1,334,299 were delivered to home bound seniors to keep them in their own homes and prevent unnecessary and premature nursing home placement!

Elder Rights Prevention and Support

Last year the legislature allocated $250,000 to assist the AAA’s in preventing and educating Iowans about Elder Abuse. The AAA’s have:

·  Elder Rights Specialists in each of the six AAA’s supporting all 99 counties.

·  Elder Rights Specialists have been trained to work with elder abuse cases providing specialized advocacy and assistance to protect and enhance the health, safety, welfare and rights of older Iowans.

·  A data collection component has been developed and is being refined to create a baseline for future monitoring and system performance measures beginning Fiscal Year 2016.
