2 / Insert the study name and the reference number. This number wasdetailed in your grant offer letter – please refer to it in all correspondence. This is a number unique to your project and helps The Corra Foundation and track information relating to your study within the system.
3 / The start date is the date you started the study.

5 / If the study has changed significantly in the focus of its delivery since your last report, please explain how and why. Any changes should have had prior approval.
6 / For this question you will need to refer back to the original workplan that you completed with your application form. Looking again at the activities outlined in your original application, please comment on the progress made towards achieving these during the reporting period. N.B whenever possible, progress to date should includetargets and numbers. Eg 10 focus meetings with the local community were delivered against a target of 8 meetings.
