MDA National

/ MDA have provided the Compass Team with a generic ‘blanket’ approval (subject to the normal terms and conditions) for any MDA National members participating in the trial (both the Pilot and Main Trial). If you are insured with MDA please contact the compass Trial and request a copy of the blanket approval.
The blanket approval refers to two policy documents which can be provided upon request or are found online at the Compass website:

Please note there are special requirements for Post Graduate and Doctor is Specialist Training categories, please refer to above link or consult you policy manager at MDA if necessary.

MIPS

1800 061 113 / Send the attached Clinical trials form to: (please consult your policy manager at MIPS if you have any questions) / Please complete the MIPS clinical trial notification form (attached) with your practitioner details. We have populated certain aspects- you just need to add your personal detailsand sign.
Please submit the form via mail or emailto MIPS (addresseson the bottom of the form).
It can take up to ten days for approval to commence recruitment. The generalinformation about the trial and ethics approval has already been sent to MIPS so you only have to send in the notification form (attached).
Please refer to your individual policy statement or contact your policy manager if you have any questions or require additional information.

MIGA

Please complete the AVANT VCS-UNSW Clinical Trial Endorsement Form (attached) and submit to AVANT.
The Compass Team have provided AVANT with information about the Trial and AVANT will simply update your policy to note that you are involved in the Compass Trial.

VMIA

/ VMIA have indicated to the Compass team that no further action is required; however you may wish to consult your policy manager to confirm your involvement in the Compass Trial. / Please consult your policy manager at VMIA if you have any questions

Clinics (from the Pilot Study)

All Clinics should notify their insurance provider that they are now participating in the Compass Main Trial. If your insurance provider requires additional information-you should provide them with the Trial Information for Practitioners Information Sheet.

Practitioners (from the Pilot Study)

If you are a new practitioner who hasn’t recruited for the Compass study before or a new practitioner has joined your clinic please refer to the Insurance Information for Practitioners and Clinics, Compass Main Trial and follow the instructions listed there.
