Instructions for the Core Curriculum Recertification Cover Sheet

1.  Enter the four letter course prefix and three digit number under which it is currently listed (example: MATH 131).

2.  Enter the complete title of the course as currently listed in the catalog. For example, MATH 131 is “Mathematical Concepts – Calculus.”

3.  Enter the name of the academic department submitting the request.

4.  Enter the name of the person submitting the recertification request. This can be the faculty member teaching the section, the department head, or other department administrator.

5.  Enter the E-mail address of the person submitting the recertification request.

6.  Enter the name of the instructor providing the class sets.

7.  Enter the E-mail address of the instructor providing the class sets.

8.  Indicate the Foundational Component Area under which the course is currently listed.

9.  Show total enrollment (all sections) of the course for each semester it has been taught over the past three academic years.

10.  Attach the completed Foundational Component Area form and the complete and final course syllabus. In addition to the minimum syllabus requirements, the syllabus should show how the core objectives were met.

11.  The person submitting the recertification request (same as number 4) must sign the form and submit it, along with the course syllabus, to the Department Head for approval.

12.  The head of the academic department under which the course is offered must sign and submit the form and the syllabus.

13.  Submit completed and signed forms (Cover Sheet and Foundational Component Area), along with the course syllabus, in electronic format to fso-ccc@tamu OR in paper format to Kristin Harper, TAMU 1125. DO NOT submit in both electronic and paper format.

14.  Submit complete course sets (one section) of student work aligned to the required State Core Objectives using the submission portal at http://corerecertification.tamu.edu. Student work should not contain grades or any instructor comments.
