Highway Access Permit
An application for a permit to construct any private or commercial driveway, or to make any revisions or additions to an existing driveway, on a State maintained highway or roadway, shall be made to the District Engineer of the Illinois Department of Transportation. A permit is required from the District office prior to commencing with the proposed driveway construction on the State maintained right-of-way.

For Non-commercial driveways: On the opposite side of this application, please fill in dimensions or distances.
For Commercial driveways: Please furnish and submit to the District Office one (1) copy of a survey plat of the property and four (4) prints of an on-site development plan at a scale of 20:1. The District Office will review the application to determine that the proposed driveway construction, the location of the driveway on the property, and the development of the property being served are in conformance with the provisions of the Department's Access Policy. Submittals will be reviewed for compliance with regulations pertaining to land usage.
A meeting may be required between the Applicant and a representative of the Department. When all requirements are met, the permit forms will be prepared for processing.
All materials and expenses in constructing the driveway are the responsibility of the property owner. The driveways are to be constructed in accordance with the Department’s Standard Specifications as noted in the terms of the approved permit.
