2011 Instructions for Nominations

Do you have an example of a business that has demonstrated what it means to “Put People First” by employing a person with a developmental disability that resulted in a great business outcome? OPWDD wants to give you the opportunity to recognize this employer.

Please provide the following information on the Nomination Form:

·  First, complete the demographic information about the business that hired the person. Provide a brief description of the nature of the business, such as the products manufactured or services performed. Most importantly, please describe how the employee’s work performance has contributed positively to the business.

·  Next, from the perspective of the nominating organization, describe the reasons why this business is being nominated and why it deserves special recognition.

·  Lastly, complete the information about the individual employee. Only one employee may be included on each Nomination Form. Please explain how the power of work has changed the person’s life.

NOTE: We would like to highlight those businesses which offer opportunities in a wide spectrum of jobs that demonstrate the diverse talents and abilities of people with developmental disabilities. Last year our winners included a café, a documents imaging company, a manufacturer of fasteners, a supermarket and a high school.
