Academic records must be sent for all secondary, continuing secondary and university-level studies. Include the cover page with each set of records sent. Include your complete academic history below. Please send a cover page for each institution attended, however, the academic history below only needs to be filled out once.

The student listed on the Transcript Submission Form is registering with the NAIA Eligibility Center. Please forward the student’s official, original academic records or certified copies of original academic records with the completed submission form directly from the school to the NAIA Eligibility Center. PLEASE DO NOT INCLUDE THIS INSTRUCTION PAGE WHEN SENDING RECORDS.

Guidelines for Official Documents

Students should be registered with the NAIA Eligibility Center before official records are sent.

The list of necessary documents varies by country. Please refer to the International Students Webpage and Frequently Asked Questionsfound at for complete details about which documents should be sent. The NAIA Eligibility Center requires school leaving certificates, diplomas, official academic records, transcripts, and records of grades for all secondary schools attended.

Academic records must be sent in BOTH the original language of issuance AND as a certified, literal English translation.

A letter from the translator including any appropriate stamps or seals explaining his or her qualifications should be included. The translator cannot be associated with the student(eg. family member) or with the athletic department of the NAIA school the student will be attending.

For high school students, if final class rank is not part of the official academic records, the high school principal or head master can send an additional letter on official school letterhead to confirm whether or not the student has graduated in the top half of the graduating class, if the student wishes to use class rank to meet two of the three eligibility criteria.

The school should send records directly to the NAIA Eligibility Center. The school can mail documents directly, or a school official canplace the official records in an envelope, stamp the flap with the school’s official seal, and have the student mail the school-sealed envelope. It must be apparent that the school official was the last person to handle the documents.

If the student only receives one copy of academic records, the school can make a copy and then certify that the copy was made from the original document by stamping and signing the copied documents.

Documents that are emailed or faxed will not be accepted. Documents sent by students without the school-sealed envelope will not be accepted.
