Instructions for Benefits Enrollment

IMPORTANT

YOU MUST READ THIS BEFORE YOU ENROLL FOR BENEFITS

We hope you are excited about joining our talented and diverse workforce. One of the key advantages of working at Prudential is the opportunity to participate in a competitive benefits program that is designed to be comprehensive and flexible.

If you are an eligible Prudential employee, you automatically receive coverage under certain plans and programs, at no cost to you (Core Benefits). In addition, you will have the opportunity to enroll in optional programs (in which you will share in the cost with Prudential) and other voluntary programs (in which you pay the full cost).

With this choice and flexibility comes responsibility. For most programs, you must make your benefit elections within 31 days of your date of employment. If you do not enroll within 31 days, you will not be able to enroll for Medical, Dental, Vision, or Flexible Spending Account coverage until the next Annual Enrollment Period, unless you experience a Qualified Change in Status. You will not have another opportunity to enroll in the LegalCare Program again until the next Annual Enrollment Period.

Please review the enclosed 2011 Annual Enrollment "Evaluating Your Options" brochure. which contains important information to help you make your benefits decisions for 2011. Your personalized Enrollment Worksheet, available online at the Prudential Benefits Center, details your health and welfare program options as well as cost information for your 2011 benefits. For more detailed information, go to “Annual Enrollment 2011,” within the “Health, Life & Wellness tab on “Inside the Rock – My Prudential” or the Prudential Benefits Center Web site at (wwwprubenefitscenter.com). Please review the following Annual Enrollment materials carefully before you select your benefit program elections for 2011:

 The Benefits Comparison — a chart comparing key components of the national medical programs.

 Benefits Program Details — important details on all of the health and welfare programs

 Health Care Program Charts — detailed summaries of the major provisions of each medical and dental program option;

 Tools and Resources — descriptions of several tools and resources available during the Annual Enrollment Period and on an ongoing basis; and

 The National Medical Program Cost Examples — illustration of how each of the 2010 national medical program works, including a side-by-side comparison of the medical program contribution amounts, benefit levels, claim processing results, and out-of-pocket expenses.

The following charts outline the benefits plans and programs available and the default coverage you will receive if you do not enroll (provided you are eligible for coverage).

If you do not enroll within the 30-day period, you may not join SESP until the next annual enrollment period.
If you do enroll, your election will remain effective unless you make a change during an election period.

Health Care and Personal Protection Programs

To make your benefit elections for health care and personal protection programs, you will need to visit the Prudential Benefits Center web site at When you visit the site for the first time, you will be asked to enter the last four digits of your Social Security number and your date of birth. You will then be prompted to create your own 8-20 digit password.

 To access the Prudential Benefits Center from your Prudential work station:

— Go to Prudential’s intranet site, “Inside the Rock,”

— Click on the “My Prudential” tab,

— Click on “Health, Life, & Wellness,” and

— Then click on the “Health, Life, Disability, and Other Benefits” ; or

 To access the Prudential Benefits Center from the Internet:

Prudential Employee Savings Plan

If you elect to enroll in PESP, you will have an opportunity to contribute on a before-tax basis, an after-tax basis, or as a combination of both. Prudential generally matches 100% of your Before-Tax Contributions, up to a maximum of 4% of your Eligible Earnings, subject to Internal Revenue Code limitations, after your one-year anniversary.

To enroll or waive participation in The Prudential Employee Savings Plan (PESP) you must use either the PESP Web site (Prudential Online Retirement Center) or the Interactive Voice Response (IVR) automated telephone system. All you will need is your Social Security Number and your PESP/Prudential Personal Identification Number (PIN). Your PIN is initially your six-digit date of birth (mmddyy). When you access the system for the first time, you will be prompted to change your PIN.

 To access the PESP Web site from your Prudential work station:

— Go to Prudential’s intranet site, “Inside the Rock,”

— Click on the “Pay and Retirement Savings” tab, and

— Then click on “Prudential Employee Savings Plan (PESP)”

 To access the PESP Web site from the Internet:

— Log on to

— Then follow the on-screen instructions; or

 To enroll, waive participation, or make changes over the telephone using the IVR, you may call 1-800-PRU-EASY (1-800-778-3279) and say the key words, “Savings Plan.”

Please note: Generally, you may access the Prudential Benefits Center, the PESP web site and the PESP IVR 24 hours a day, seven days a week, except during scheduled maintenance. However, you will not be able to access them until your employment record has been created and you have authenticated your access as a first-time user. It is important to remember that your health and welfare password and PESP PIN are two separate identifiers. They are used for different accesses to complete your benefits enrollment.

[1] Short Term Disability coverage (except for certain state-mandated benefits) is not available to Agency Distribution Sales Professionals with an adjusted service date of 1/1/05 or later, and part-time employees.
