Instructions for Application for ISO 9001 Co-certification from the BSMI

These Instructions are provided to explain the application procedures for ISO 9001 certification from the Bureau of Standards, Metrology and Inspection (BSMI), via foreign certification bodies that have signed mutual recognition agreements (MRAs) with the BSMI.

  1. Terms:
  2. The cooperation partners of the BSMI are referred to as “Partners.”(Contact information as Appendix I)
  3. Organizations located in other countries are referred to as “Applicants.”
  4. Application Procedures: (flowchart as Appendix II)
  5. Applicants must have obtained the ISO 9001 registration from the Partners in advance.
  6. The Applicant must complete the BSMI application documents, including “Form QA-90”, “Form QA-91”, and “the Declaration of Rights and Obligations for Management Systems Registration”. When a Taiwanese agent is authorized, the Applicant must also complete “Letter of Authorization” (QA-95) and file the previous application documents to the Partners. These application documents can be downloaded from our web site at http://www.bsmi.gov.tw/english/index.htm.
  7. After accepting the application case, the Partners shall send the completed application documents together with the following documents to the BSMI: (All the documents shall be written in English or in Chinese.)

(1) A copy of the ISO 9001 certificate. (The Certificate must be granted by the Partner that conducted the relevant audits. Certificates granted by the Partner via document review through other cooperation agreements are not acceptable.)

(2) A copy of a full clausal surveillance/re-assessment/initial assessment audit report with the relevant audit plan and corrective action plan for the non-conformities within one year.

(3) Copies of the assessors' resumes including the NACE (Nomenclature generale des activites economiques dans les communautes Europeennes) Code.

(4) Information on ISO 9001 Audit Time (Appendix III).

2.4. The BSMI shall make a final decision on whether the certification is approved. If any problem arises during the application review, the Partners shall provide supplementary information or documents for BSMI approval. If there is still a reason of lack of approval, the BSMI will not issue a certificate.

2.5. After the documents have been reviewed and the decision of whether to grant certification has been made, the BSMI will notify the Applicant to pay the related fees, which will be exchange to the US dollars according to the exchange rate of the Customs of the Republic of China then. The fees are listed below.

(1) Annual fee: NT$12,000 per case.

(2) Application fee: NT$10,000 per case.

(3) Certificate fee for new applications: NT$2,000 per case.

2.6. Once the Applicant has paid the fees, the BSMI will notify the Partner and issue the certificate.

  2. The surveillance/re-assessment audit of the co-registered organization shall be conducted by the Partners. The frequency of surveillance audit per year depends on the requirements of each Partner. The BSMI may assign assessors as observers to join the audit of the co-registered company conducted by the Partner, when necessary.
  3. After surveillance/re-assessment audit, the Partners shall send the following documents to the BSMI in a timely manner: (All the documents shall be written in English or in Chinese.)

(1) A copy of the surveillance/re-assessment audit report, including the relevant audit plan and corrective action plan for non-conformities.

(2) Copies of the assessors' resumes including the NACE (Nomenclature generale des activites economiques dans les communautes Europeennes) Code.

(3) A copy of renewal ISO 9001 Certificate issued by the Partner.

3.3. Issue/re-issue of the BSMI ISO 9001 Certificate for Co-registration

3.3.1. The expiry date of the BSMI ISO 9001 Certificate for the co-registered company agrees with that of the ISO 9001 Certificate issued by the Partner.

3.3.2 The BSMI will notify the co-registered company about the renewal of the BSMI ISO 9001 Certificate according to the previous documents described in 3.2 before the impending expiration, or when a change of company name, a change of company address or reducing the scope of registration. The fee of the renewal certificate is NT$500 per case, which shall be paid before the Certificate re-issue.

3.3.3 In the case of extension to the scope of registration, the co-registered company, or the Partner shall formally notify and send the previous documents described in 3.2 to the BSMI. The “Letter of Authorization” is also necessary, when the authorized Taiwanese agent assists in handling these matters. The fee of the renewal certificate is NT$500 per case, which shall be paid before the Certificate re-issue.

3.3.4 In the case of disappearance of the ISO 9001 Certificate, the co-registered company shall formally notify the BSMI for re-issue the current Certificate. When the authorized Taiwanese agent assists in handling this matter, the “Letter of Authorization” is also necessary. The fee of the renewal certificate is NT$500 per case, which shall be paid before the Certificate re-issue.

4. Changing Status:

4.1 Companies that are currently directly registered with the BSMI may apply to the Partners for changing their status to co-registered companies.

4.2 Applicants must apply to the Partners for ISO 9001 co-registration two months prior to the scheduled surveillance audit by the BSMI.

4.3 The Partners shall send the following documents to the BSMI: (The documents shall all be written in English or in Chinese.)

(1) A letter from the Applicants indicating their intention to change the status to co-registration.

(2) Copies of the surveillance/re-assessment/initial assessment reports of that year, including the relevant audit plan and corrective action plan for non-conformities.

(3) A copy of the ISO 9001 certificate. (The information on the name and address of the company in the certificate must be the same as those appearing in the BSMI certificate.)

(4) Copies of the assessors' resumes including the NACE (Nomenclature generale des activites economiques dans les communautes Europeennes) Code.

4.4 After accepting the application, the BSMI will review the documents in accordance with clause 2.4 to 2.6. If a BSMI ISO 9001 certified company succeeds in the change of status into a co-registered one, the Partner shall replace the BSMI for conducting the surveillance/reassessment audit.

5. Cancellation and withdrawal of the BSMI ISO 9001 Co-registration

5.1 When a co-registered company intends to cancel the BSMI ISO 9001 Co-registration, the company or its authorized Taiwanese agent shall formally notify the BSMI.

5.2 A co-registered company must pay the relevant fees for the BSMI ISO 9001 Co-registration on time. In the case of a delay of a payment, the BSMI will formally notify the co-registered company about the payment again. If the company still fails to pay on time again, the BSMI shall withdraw the co-registration of the company.
