Innovation Pathway Designation Final Application—Instructions for Submission

Submission in WizeHive: You will be submitting your application in the WizeHive portal, as in the preliminary stage. The main change from that stage is that, with the exception of basic contact information and assurances, the submission will be done by upload, as described here, rather than through text boxes in a web-based application.

The link for WizeHive will be sent to you by separate email. You will be able to navigate to the main Application page, where you will enter partner information on line, and then proceed to the upload process.

Note: Please make sure not to include any individual student level data in your materials.

  • Uploading the Narrative: Attached is a newshortened Word version of the Application template that you received in December. The questions are identical to those provided earlier, but much of the explanatory text has been removed.

Please use this template by entering or inserting your responses in the designated spaces. This document will be characterized as the Narrativefor your application. Please keep your responses to each question in Arial 12pt. font, single spaced, double spaced between paragraphs. There is no word limit for the questions.

Abstract: We’ve included space at the beginning of the Narrative for an Abstract. Please briefly summarize your application here, and limit your response to 500 words. Note that we will not be evaluating this part of the application; it is simply an opportunity for you to introduce the application for reviewers.

Please use the following naming convention for theNarrative:

“[name of Lead Applicant] IP Final App Nar”

For example, the narrative from Everett Public Schools would be labeled:

“Everett IP Final App Nar”

The final version of your Narrative should be convertedto a PDF file prior to submission to ensure that the integrity of the document is maintained. There will be a designated “upload box” for the Narrative at the start of the on-line application.

Note: The only substantive change in the Narrative is there is no longer a requirement for the submission of a budget for the designation. After careful consideration, it has been determined that that information is not required for the review process, though it will have been needed for your internal planning effort.

  • Uploading All Other Materials:

We are providing an Index Templatein which you will list all the documents and files you are presenting, to assist our reviewers match uploaded documents to their related questions. There will be a designated upload box for the completed Index template right after the Narrative box.

The WizeHive format will then provide a series of generic upload boxes, each of which will be able to accept one file. Your submissions should correspond to the list provided in your Index. In order to be able to link all other materials to the questions to which they respond, we ask that you use the following naming convention, bylabeling them according to the question to which they apply, in the order in which we should review them. The naming convention for any upload is: the number of the Guiding Principle as it appears in the Application, then the Question letter and section, and then a brief name for the file or document. For example:

“1_ A_ Program growth projections”

“1_ B_Written admissions policy”

“1_ D_Recruitment Plan”

“2 _A_Manufacturing_Scope and Sequence”

“2 _F_Sample_Schedule”

The leading numbers for the sections should be as follows:

1 – Equity and Access

2 –Guided Academic Pathways

3 -Enhanced Student Support

4 -Connection to Career

5 -Effective Partnerships

6 –Day in the Life

7- Staff Plan and PD

8 – Grant Application

It is acceptable for there to be more than file per question; kindly use the same leading number and letter, but give them different descriptive names. Also, not all questions require an attachment, so in that case, simply skip use of that question letter.
