I, ______, (client) understand that massage therapy provided by, ______(massage therapist,) is intended to enhance relaxation, reduce pain caused by muscle tension, increase range of motion, improve circulation, and offer a positive experience of touch.

The general benefits of massage, possible massage contraindications and the treatment procedure have been explained to me by the therapist. I understand that massage therapy is not a substitute for medical treatment or medications, and that it is recommended that I concurrently work with my Primary Caregiver for any condition I may have. I am aware that the massage therapist does not diagnose illness or disease, does not prescribe medications, and that spinal manipulations are not part of massage therapy.

I have informed the massage therapist of all my known physical conditions, medical conditions and medications, and I will keep the massage therapist updated on any changes. I understand that there shall be no liability on the practitioner’s part due to my forgetting to relay any pertinent information.

If I experience any pain or discomfort during the session, I immediately communicate that to the therapist so the treatment can be adjusted.

Cancellation/Late Policy

Should you need to reschedule or cancel your appointment, we require 24 hours notice. If 24 hours notice is not given, you may be charged for the appointment time that you missed. We recognize that emergencies occur; therefore, we reserve the right to make individual decisions regarding assessing charges for missed appointments.

Initial

If you are late arriving for your appointment and have not made arrangements with your therapist, the length of yoursession may be reduced, as late arrivals impact the other appointments scheduled that day.If yoursession is shortened because of your late arrival, you are still responsible for full payment as if the full session was performed.

Repeated no shows or cancellations will require complete prepayment to schedule.

Initial

Our therapists are independent contractors, and as such, are paid only for the work they perform. No shows and late cancellations directly affect their pay.

These policies are for your benefit as well as for the benefit of your therapist, and we appreciate your adherence to them.
