Incident Reporting

As a Trust we have a duty to report any incidents or near misses. The Incident Reporting policy states that incidents should be submitted to the Patient Safety or Health & Safety Team within 5 working days from when the incident occurred.
Core Standard C1a from the Healthcare Commission's Standards for Better Health states:
"Healthcare organisations protect patients through systems that: identify and learn from all patient safety incidents and other reportable incidents, and make improvements in practice based on local and national experience and information derived from the analysis of incidents".

It is necessary to categorise all incidents. We encourage staff members reporting the incident to do this on the relevant part of the incident form. The two code books below will allow each incident to be categorised dependent upon the nature of the incident. These were revised in April 2009 and will be reviewed every 12 months.
Patient Codes- This should be used tor all incidents affecting patients
Non-Patient Codes - This should be used for all non-patient related incidents.

How to order - Please contact your site co-ordinator in the first instance who should have a supply of incident forms and continuation sheets. If there are any issues, please contact Adam Short, Governance Information Manager - 01773 525099 ext 5569 who will be able to advise further.
All Patient Safety incident forms need to be sent to the Patient Safety Team at Walton Hospital once they have been signed off by the line manager
All Non Patient incident forms (e.g. staff incidents/fire incidents etc) need to be sent to the Health & Safety team at Babington Hospital once they have been signed off by the line manager.

Electronic Incident Reporting
DATIXweb - The PCT has piloted DATIXweb (electronic incident reporting) within Learning Disabilities Service and at WhitworthHospital during 2008. The evaluation of this process is currently being undertaken with a view to rolling out DATIXweb throughout the PCT. Further details will be made available at a later date.
Reports- It is possible to requestinformation relating to incidents that are stored on the DATIX Risk Management system by completing the report request form. This information can then be emailed to Adam Short for processing.
NPSA Benchmarking Reports:
Every 6 months the National Patient Safety Agency (NPSA) produce reports that compares patient safety data of our organisation against other similar organisations. These reports along with supporting papers can be accessed below:
