The successful applicant’s responsibilities would be as the Lead Consultant Psychiatrist of a multi disciplinary team providing a general adult service for Rehabilitation patients in the Rehabilitation Service based at Kelvin(12 beds) and Clyde(18 beds) Ward at Gartnavel Royal Hospital. The clinical work is in-patient based but links with the wider network or community services need to be developed.

The post is based in Glasgow which provides a wide range of well established mental health services for a diverse population. The city itself offers an excellent quality of life with good links to the wider UK and Europe with excellent schooling and leisure opportunities.

The successful candidate would be encouraged to take opportunities available for further education and continuing professional development. There are active education and audit programmes within the service.

Applicants must have full GMC Registration, a license to practise and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher specialist training leading to CCT or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from the date of interview.   1. Current Psychiatric Services – West Glasgow Area

The current post is based in the North West locality of the Glasgow City Health and Social Care Partnership(HSCP).There are 6 HSCPs across the NHS Greater Glasgow and Clyde(NHS GG&C) Health Board Areawhich have recently been established to provide health and care services to the population. The Head of Adult Services responsible for mental health services is Mr Colin McCormack and the Clinical Director in Psychiatry is Dr David Gerber.The West Glasgow area also includes the West Dunbartonshire and Lomond areas. The Head of Service for these areas is Mrs Julie Lusk. The Associate Medical Director for Mental Health is Dr Michael Smith.

The North Westlocality serves a population of around 210,000 based on 2001 census data, with an additional 60,000 at the Lomond area. There are established links within both local authority areas through HSCP’s and excellent joint working arrangements exist. Community services are well established in NHS Greater Glasgow and Clyde and based around community mental health teams. There are five adult resource centres in the North Westlocality and West Dunbartonshire: Riverview, Arndale, Goldenhill, Shawpark and Riverside, with an additional team in Helensburgh. The staff within these services constitute the North West Division of Psychiatry. These services, in recent years, have been enhanced by the development of Crisis Services and Primary Care Mental Health Teams.

In-Patient services are based at GartnavelRoyalHospital. This is a modern facility constructed in 2007, which provides state of the art accommodation for service users. There are 6 wards; 3 acute adult wards, 1 IPCU ward and 2 older adult wards. There are 117 acute beds provided in modern single rooms all with en-suite facilities. In addition, there is a rehabilitation service, an Addictions ward and day Unit and a Learning Disability In-Patient facility on the campus. The in-patient service is actively supported by Occupational Therapy Services, Dietetics, Physiotherapy, Advocacy and a well established patient activity service. There is a strong ethos of positive engagement with third sector organisations and service users.

There is an established Liaison Psychiatry service across the Health Board area. Patients in NHS GG&C have access to specialist Psychology, Psychotherapy and Addiction services. The NHS GG&C Perinatal Service, based at Leverndale hospital, provides board wide services and the ESTEEM service provides Early Intervention Psychosis services. There is also a well structured Forensic Psychiatry service providing community, in-patient and Medium Secure services to the Board area. The Community Eating Disorder Service is another Board wide service based in North East Glasgow with admission beds at StobhillHospital. There is also a dedicated Out-Of-Hours Service across the city providing psychiatric nursing assessment and support from 8pm-9am.

  1. Duties of the post

The Rehabilitation Service at Gartnavel Royal Hospital (GRH) is a Multi disciplinary Service with 2 adjacent in-patient Units; Kelvin and Clyde wards.Historically, Kelvin ward has functioned as a 12 bedded unit offering a full range of rehabilitation services, with the 18-bed Clyde Ward functioning as a complimentary slow paced rehabilitation and long-term complex care unit. Recent developments in UKservice guidelines for the provision of rehabilitation services have led to a drive for reshaping GRH rehabilitation services to be more aligned to contemporary models of rehabilitation. This work is on-going and the successful candidate would be joining GRH rehabilitation services at an exciting point in the service’s evolution.The successful candidate would have a key role in service-development.

Both wards are supported by a multidisciplinary team consisting of nursing staff, rehabilitation liaison practitioner, weekly general practitioner input, occupational therapy, a clinical psychologist and dedicated administration staff. Therapeutic work includes development of adaptive daily living skills, minimising the effect of challenging behaviour on social inclusion, and psychological interventions (e.g. Behavioural Family Therapy). Links with community mental health services are maintained (e.g. through CPA).

The population accessing rehabilitation services typically havelong-term diagnoses of schizophrenia, bipolar disorder or other psychoses (e.g. delusional disorder) and are typically subject to compulsory measures under the Mental Health (Care and Treatment) (Scotland) Act, 2003. They are likely to have the following needs:

1)Ongoing (e.g. positive and negative syndromes) psychotic features (often resulting in high-dose antipsychotic treatment)

2)Difficulties sustaining community residence (recent extended duration of hospital admission, high frequency admissions, recent loss of a supported living environment)

3)Vulnerabilities due to cognitive impairment, difficulties engaging with services, risk of harm to self/others, self-neglect, difficulties with motivation and daily life skills, risk of exploitation, and/or complex physical health problems.

Rehabilitation services are unique in respect to the length of time that the service has to work with the person towards recovery, and historically length of admission has included a subgroup with lengthier admissions (e.g. > 5 years). Achieving improved flow through the service is a priority, thereby enabling improved access to rehabilitation services for those in acute beds at GRH in a timely and responsive fashion. Discharge pathways are typically to supported accommodations or independent tenancies with support. Rehabilitation service aims include maximising quality of life post-rehabilitation and contributing to relapse prevention.

The post’s main responsibilities are to deliver Consultant Psychiatrist Medical input and leadership into the multi-disciplinary team. The work is predominantly based in the 2 Rehabilitation wards (Kelvin and Clyde). There is additional assessment work and liaison with the acute General Adult Wards at GartnavelHospital who are the sources of referral to the Rehabilitation Service. There is no specific Community or Out-patient work associated with the post, however it is expected that the postholder might develop links with CMHT’s and Community based Rehabilitation resources in particular e.g. Supported Accommodation Units to help develop an holistic service.

There is an additional session (1PA) to provide cover for the IPCU ward at GartnavelRoyalHospital. The Rehabilitation post and IPCU post are involved in cross cover arrangements for leave. In addition, the current IPCU Consultant is not employed on a Wednesday, therefore this session is provided in recognition of the work that might arise from this arrangement.

There is usually a junior doctor allocated to the post with shared responsibilities between Rehabilitation and IPCU. In addition there is a funded General Practitioner providing weekly input to the Rehabilitation wards.

Applicants must have full GMC registration, a licence to practice and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher specialist training leading to a CCT in General Adult Psychiatry or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from the date of interview. Non-UK applicants must demonstrate equivalent training.

It is desirable that a successful applicant would be an approved medical practitioner in terms of the Mental Health(Care and Treatment) (Scotland) Act, 2003 or be eligible to apply. It is essential that the successful applicant has membership of the Royal College of Psychiatrists or equivalent and has a minimum of 3 years Post Graduate experience working in Psychiatry.

This job plan is negotiable and will be agreed between the successful applicant and the Clinical Director. NHS Greater Glasgow & Clyde initially allocates all full time consultants 10 PAs made up of 9 PAs in Direct Clinical Care (DCC) and one core Supporting Professional Activities (SPA) for CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings. The precise allocation of SPA time and associate objectives will be agreed with the successful applicant and will be reviewed at annual job planning.

Continuing Professional Development

The post holder will be expected to maintain good professional standing for CPD with the Royal College of Psychiatrists. There is a study leave budget and medical staff are encouraged to ensure that they keep their practice up to date. In addition the post holder will be expected to take part in the annual cycles of peer review, appraisal and job planning.
