What is Project Access?

Project Access is a national effort funded by the Maternal and Child Health Bureau of the Health Resources Services Administration (HRSA). Through Project Access, the Epilepsy Foundation of Florida (EFOF) aims to:

Improve communication between primary care doctors and specialists.

Eliminate delays in getting appointments.

Empower parents to be co-decision makers and to take a more proactive role in their child’s care.

Spread the concept of the Medical Home. A medical home is a primary care setting that is patient-centered, includes the family as a partner in treatment decisions and utilizes a care coordinator to manage a patient’s health care.

Maximize the resources available for health care coverage and primary care access to all children with epilepsy regardless of socio-economic and immigration status.

Educate families and the community about epilepsy.

How do we help our families through Project Access?

Project Access is designed to provide parents and caregivers an easily manageable system for maintaining current medical information for their child with 3 distinct parts. Our goal is to keep your child’s care team informed and involved with your child’s needs through the use of the following tools:

ALL ABOUT ME CARE NOTEBOOK

A binder that includes the following:

Care Plan– A patient’s basic personal and medical information, includinga Seizure

Action Plan, which shows how to deal with a seizure in emergency situations

Seizure Log – A seizure tracking record

Medication List – A list of current and formermedications and side effects

Medical History– Anorganizational section for important, recent medical records

Family Resources – A list of local, state and national resources for families with

children with epilepsy, and other useful tools for these families

FLASH DRIVES

A data storage device that provides health care providers and emergency personnel ready access to the most current and accurate neurological and medical records including the latest: EEGs, consultation notes, imaging, blood work, and psychological evaluations, etc.

MEDICAL ID CARD

A wallet-sized card designed to be carried by the child in his or her school back pack or wallet as a handy and reliable medical summary for school or emergency situations. The Medical ID card is a safe and reliable back up for reducing potential allergic reactions/interactions, intolerances or side effect profiles.

The goal of EFOF is to enhance the continuum of care for children and youth with epilepsy.
