I will participate in the programme and I will pay after receiving the invoice electronically by ZETA Company s.r.l., VAT Number RO1883813, registered in Register of Commerce under number J13/3018/1992in IBAN account RO88 EGNA 1010 0000 0061 6885 opened in Marfin Bank s.a., Constanţa Branch, amount of 760 EUR beforeOctober 5thor1140 EURthereafter.(For all amounts in this form V.A.T. will be added - 20%; this is not applicable if my company is registered in The Register of Intra Community Operator.)
1 - After payment, within 24 hours, I will inform the person who issued the invoice to me by e-mail or phone.
2 - If I will not pay the bill within that time, I accept that the place for me to be occupied by another participant, and the bill will be written off.
3 - If I notice more than 30 days before the non-participation during which we completed this Form, I agree to participate in the next course on the same subject (date to be announced at least 30 days before). Participation at another time involves paying an additional fee of 1410 lei.
4 - If I renounce to participate in this course with less than 45 days before, I will notify in writing the person who issued the invoice to me. In this case ZETA Company s.r.l. will retain 1950 lei (organizational costs).
5 - If I do not participate in this course and after I paid in full, without written notification (via email) of the person who issued the invoice, ZETA Company s.r.l. shall withhold the total amount.
6 - In case of postponement of the course, the participant can choose whether to attend the next course or to receive entirely amount paid. This option is not applicable to situations described in paragraphs 3, 4 and 5 from above.
This Course Registration Form becomes valid only after the full payment.
This Application Form substitutes the Contract between the parties.
