Directions for Participant Statement of Participation
Each participant should fill out one of these forms upon completion of the activity. If there are multiple sessions then the form is filled out at the end of the last session and all session dates are included on the form. The PI / PD assistant completes information as follows:
Once the assistant completes the form, the PI / PD can print as many copies as there are participants.
Directions for Sign-In Sheets
There are 2 forms available for use
1) (Template_TypedNames) A sign-in sheet where the names of participants are typed on the form and the participant must only sign the sheet to verify their attendance.
2) (Template_PrintedNames) A sign-in sheet where the PI does not know who will be attending.
The typed name sheet is preferable and should be used when participants will be attending multiple sessions. If the names are alphabetized it will be much more efficient for participants to find their names.

Typed Name sheet
The names of chosen participants should be typed in prior to the session. This is the preferred form for a project that will involve participants in multiple sessions. Here is what the PI assistant should do to use this form:
Note - once you have typed in all participant names and the project name, you should save the sheet. If you have multiple sessions, then type in the first date, print the sheet; replace the date with the date of the second session and print the sheet. By using this approach you can print a sign-in sheet for every session by just changing the date and printing a copy.
Printed Name Sheet
If the participants are signing-in and you do NOT have a printed list use the “Printed Name” sign-in sheet. Here is how to use that sheet:
