BELGIAN WORK PERMIT and VISA application(stay +90 days)
Applicant to be present in person with this checklistand all the following required basic documents:

Step 1: BELGIAN WORK PERMIT APPLICATION IN BELGIUM
Requested by the Belgium-based employer
Step 2: VISA APPLICATION
3 visa application forms(inDutch / French / English)properly completed, signed and dated.
4 recent passport-sized photos (3,5cm x 4,5cm) in color with a white background.
Original Passport (+2 copies). It should be issued less than 10 years ago and valid for at least 12 months at the point of visa application.
Original Malaysian resident permit (+ 2 copies)
Employment pass / Work permit / Student pass / Permanent Resident card/valid for at least 6 months.
Original Belgian work permit (+ 2 copies)
Original Employment contract (+ 2 copies)signed by both parties
Original Certificate of good conductissued less than 6 months and covering the last 5 years,by the relevant authority of the country of residence or country of origin and legalized by the Embassy of Belgium, Kuala Lumpur.
Medical certificate issued less than 6 months. The original of this certificate is required for your work permit application in Belgium. It must be signed and stamped by the medical clinic designated by the Embassyand legalized by the Embassy before you can send it to youremployer in Belgium.This is valid only 6 months from the date of issuance and it is advisable to keep a copy for your own reference.
Extract of Birth certificate (no photocopies). The extract must be legalized by the Malaysian Ministry of Foreign affairs and legalized by the Embassy of Belgium, Kuala Lumpur. For non-Malaysian, it needs to be legalized by the Ministry of Foreign affairs and the Embassy of Belgium from the origin country.
Residence address in Belgium/ Rental Contract (+2 copies).
Your flight reservation (one way).At the collection of your visa: your confirm flight ticket (original+ copy).
Proof of travel/accident insurance to cover possible medical expenses, hospitalization costs and/or repatriation for medical reasons while travelling in Belgium or in other Schengen countries, with medical cover for at least € 30.000/RM140.000. You will have to show proof of travel/accident insurance when entering the Schengen area.(+2 copies).
Immigrationhandling fee : The contribution covers the administrative costs involved with the processing of a visa application, a residence permit application or a demand for a change in status.The proof of payment will have to be submitted along with the application at the time of the application. Please refer the dofi website , under Contribution covering administrative costs of an application to see the amount you have to pay and the details of the bank and payment (not refundable and subject to change)
