ICE STUDENT AGREEMENT – COLUMBIA CITY HIGH SCHOOL

The ICE Work-Study Program is planned to develop a student-trainee academically, economically and socially. In doing this, there are definite goals that must be accomplished. There are responsibilities the student-trainee must realize, must agree to cooperate with and must carry out to the fullest extent in order to receive credit for the course.

As a student-trainee, I understand and agree to adhere to the following rules:

  1. To be regular in attendance and on time at school and on the job.
  1. To notify Mr. Clark and my employeron the day that I am absent from school and from my training station. If I am absent from school, I must also be absent from work on that day unless approved, in advance, by Mr. Clark. Failure to call in will result in disciplinary action as stated in the Columbia City High School Student Handbook.
  1. I may be dismissed from the program for more that seven (7) absences from school for the entire year.
  1. To perform assignments in the ICE related class with sincerity (meeting all deadlines) and maintain passing grades in all other classes.
  1. To realize that I am under the jurisdiction of the school, whether in school or at my place of employment and will abide by all school rules and regulations. (i.e. No smoking or chewing on the job, etc.) If certain behavior is not allowed at school, it is not allowed at my training station.
  1. If I am required to leave school because of any disciplinary reason, I understand that I cannot report to my training station.
  1. To conduct myself in a satisfactory manner, both on the job and in the classroom, reflecting credit on myself, the ICE Program and Columbia City High School – (Maintaining confidentiality, honesty, and good work ethics). Failure to do so will result in my removal from the training station and loss of credits. If I am fired from my job, I realize that I am no longer a fulltime student and will be removed from school for the remainder of the trimester.
  1. To attend the ICE Employer Appreciation Banquet with my employer-supervisor as my guest and to pay a banquet fee of $35, this will cover the cost of a plaque and both meals. The $35 may be paid in a lump sum or in payments of $5 per month starting in September.
  1. To practice safety in school and on the job as well as to observe all traffic regulations and school policies in the use of a car (vehicle) as transportation to and from my training station.
  1. To know that it is my responsibility throughout the year to be well dressed and groomed both in school and on the job.
  1. To work with and cooperate with the teacher-coordinator as the recognized administrator for any adjustments or changes in my training. I will not quit or change training stations without first discussing alternatives with Mr. Clark who will converse with my parents/guardians. A mutual agreement among all participants in the program will be confirmed.
  1. That as an ICE student, I am not eligible for unemployment benefits at the conclusion of my training period.
  1. Drug testing may occur at the work place. Failing a work-related drug test constitutes violation of CCHS drug policy as stated in the CCHS Handbook. Loss of job falls under item #9.

I fully understand the above statements, and I agree to cooperate in carrying them out to the fullest extent.

I hereby consent to allow our son/daughter to become a student-trainee of the school’s ICE program. The aims and policies of the programs have been explained to us; we understand our responsibilities as parents of a student/trainee; and we will help our son/daughter fulfill his/her responsibilities in the ColumbiaCityHigh School’s ICE program.
