It is the assessment of this social worker that insert first names and last name of family are suitable for the upbringing of children and meet the State of Florida Pre-adoptive requirements.

The adoptive applicants, insert first names and last name of family meet the requirements to adopt from China (as posted on the CCAA website [dated 10.10.05] under the heading “What requirements should the adopters meet?”). They are over the age of 30 years, they have the capacity for rearing and educating an adoptive child, and they do not have any disease that would make them considered medically unfit for adoption. They will be adopting an abandoned child from a social welfare institute.

Home Study Preparer’s Authority to Conduct Home Studies, Declarations, and Signature

Since the State of Florida does not require third party verification, this home study report is being sent directly to the United States Citizenship and Immigration Services for processing.

Insert preparer name (and title) if applicable, certifies that insert he or she as applicable J is authorized under 22CFR, part 96,to complete home studies for Convention adoption casesthrough Chinese Children Adoption International, which is a fully licensed agency with the authority to complete home studies, and is licensed through the State of Florida, certificate #18-3022-000-08 with an expiration date of December 14, 2009. This worker is additionally approved to perform home studies under the license of Chinese Children Adoption International, Inc., a private child placement agency licensed by the State of Florida Department of Children and Families to place children for adoption and to conduct home studies in Florida.

Chinese Children Adoption International, Inc. is accredited to provide adoption services in connection with adoptions under The Hague Convention Protection of Children and Co-Operation in Respect of Intercountry adoption. The certificate of accreditation was issued 4/1/08 and expires on 4/1/12. The agency’s license number is 83179 and is renewed annually. The current license is valid through September 2009.

In accordance with 8 CFR 204.311(f) the home study preparer declares under penalty of perjury under United States law that:

1. The home study preparer personally, and with professional diligence reasonably necessary to protect the best interests of any child(ren) the applicants might adopt, conducted the home study (including personal interviews, home visits, and all other aspects of the investigation needed to prepare the home study report).

2. The factual statements in the home study are true and correct to the best of the signer’s knowledge, information, and belief.

3. The home study is a true and accurate copy of the home study that was provided to the prospective adoptive parents and immigration.

4. The home study preparer has advised the applicants of the duty of candor and has informed the adoptive applicants of their duty to disclose all information, and the consequences should they fail to disclose any information for questions asked. Further, this worker has advised the applicants of their duty of candor under paragraph (d) of this section, specifically including the on-going duty under paragraph (d)(2) of this section concerning disclosure of new events or information warranting submission of an updated or amended home study.
