Reasonable Accommodation Approval/Denial (RS-8) from Property Supervisor. Be sure that page 2 is completed and a copy is returned to your Supervisor within 30 days. Attach ALL relevant paperwork – work orders, etc.
Options for Applicants/Residents with Disabilities (RS-9), Verification of Need for Special Accommodation (RS-10)
All adult members including Guarantor of Lease: ScoreCard showing the final decision. Include “Print Activity” Also, ifthe final decision is “Approved with Conditions”: Include “View Decision” report to reflect authorized conditions of move in.
