*Mobile Weather Station Placed on NCS Vehicle at Lease Entrance, this is where all weather conditions were taken after JSA was completed.

Summary:

On February 4th 2016 NCS performed a Well HSE (Health, Safety, and Environmental) and Air Quality Inspection of the XXX, Section XX. This report includes all Hazards, Potential Hazards, and Gas Leaks that our certified team found on location. The Report includes where the area of concern is and what the concern is. This report also includes where VOCs (Volatile Organic Compounds), HAPs (Hazardous Air Pollutants), or Methane leaks are found on location. We also have included infrared video and / or picture of the leak. On your location we marked the gas leaks with bright orange marking ribbon.

Before any work commenced all the proper PPE was put on at lease entrance, a JSA was filled out, and weather conditions were noted and monitored. First we did a walk around location with only the proper PPE and our gas detector, not the Infrared gas recorder. This is when the HSE Location Inspection Report was filled out. We found 9 hazards. The hazards found are as follows:

-1. No well sign or legal description anywhere on location or at entrance of location

-2. Only one set of stairs leading to the Fiberglass water tank

-3. No Stairs over the containment of the Tank Battery Facility

-4. The Containment in the Tank Battery is a little tore up

-5. Only Dirt Containment around the extra water tank, the Fiberglass water tank

-6. There is some extra drill pipe on the west side of location, also lots of garbage all around location

-7. There is a camper parked on the west side of location, out of the way but just sitting there

-8. There is a filter sock laying in the dirt berm for the Treater, its laying in the dirt on the NW corner of the berm

-9. The Wellhead Cellar is full of fluid, should be sucked out and taken care of

These Hazards are noted on the attached location HSE report.

After the walk around was finished the GF320 Camera with the specialized extra magnification lens was taken out and used to view location. With the 14.5 degree lens we are able to stay back 75 feet from everything on location or be off of location to get the correct angle while still looking at every area on location that has a potential to have a gas leak. We found 12 gas leaks on location. They are as follows:

-1. The Treater shack had gas coming out of it when the door was opened. This is due to the back pressure regulators.

-2. Oil Tanks 3 & 4 where leaking out of the thief hatch (The two tanks closest to lease entrance)

-3. Both Water tanks were leaking out of the thief hatch. The worst tank was the Fiberglass tank, it had a more significant leak and also the Electronic tank monitor was leaking as well where it threads into the tank

-4. Found 6 leaking Vick clamps on the vent line leaking. The pressure was high due to a restriction and this could have been the cause for a lot of these leaks and some of the leaks on the tanks

-5. The flare for the vent line was sending out a small amount of gas but it was not lit. Also the red light on the ignition source was on.

Total Number of gas leaks found: 12

These leaks are marked on location with bright orange marking ribbon on location and also noted in the attached Air Quality Report.
