How to Nominate a Candidate for the

Long Island Mathematics Educators Hall of Fame

Candidates can be nominated in any one of three ways:

1. By a colleague who has extensive knowledge of the candidate and who can complete the nomination form:

a. Download the nomination form from the website ncmta.net.

b. Complete the nomination form either electronically or hard copy.

c. Mail completed form to Mary Ann Mansfield at or by post to Mary Ann Mansfield, LIMEHOF Nominations Committee, 5 Birchwood Court, Suite 3C, Mineola, NY 11501.

2. The name of the candidate is suggested by a colleague or LIMEHOF committee member who does not have extensive knowledge of the candidate.

a. The nominator will complete the form as best he/she can. The narrative summary is required.

b. The candidate will be notified of his nomination and follow the procedure:

1. Download the nomination form from the website ncmta.net.

2. Complete the nomination form either electronically or hard copy. (INDEPENDENTLY, PARTS OF THE NOMINATION FORM WILL BE FORTHCOMING FROM THE NOMINATOR, SPECIFICALLY THE NARRATIVE SUMMARY.)

c. Both the candidate and the nominator mail completed forms to Mary Ann Mansfield at or by post to Mary Ann Mansfield, LIMEHOF Nominations Committee, 5 Birchwood Court, Suite 3C, Mineola, NY 11501.

3. By the candidate him/herself.

If an individual feels that he/she has the qualifications for nomination, but has been overlooked by the committee, he/she is invited to download and complete the nomination form. However, a supporting letter from a colleague in the mathematics community must accompany the nomination.
