How to Create a Student Evaluation

How to create a Student Evaluation:

  1. The Student Evaluations website can be found at: Log in with your Net ID and UoA password. If you do not know these or have problems with your login details, please contact the Service Desk on ext 85100.

  1. The courses you are associated with should be listed here. If you cannot see these, then please contact the Cecil Service Desk (ext 85100 option 2). You may choose to see all courses, past courses or active courses. You will be able to edit these and also go in to print out survey reports.
  2. Click on the appropriate course then on the Create button.
  1. Indicate if the evaluation is included in the evaluation plan.
  1. Choose the type of evaluation you wish to administer, and then click ‘Next’. All evaluations consist of some generic questions with the option to add more from the supplementary question bank, up to 18 questions in total. The Formative Evaluation forms have no generic questions listed; you will be able to choose up to 18 questions from the question bank. Click on theNextbutton.

  1. Choose from the options.
    To deliver the evaluation in class, ensure that ‘Print for offline delivery’ box is checked,a PDF version of the evaluation form will be available for download at the end of this process.
    To deliver the evaluation online through Cecil, check the ‘Deliver online’ box, an announcement can also be sent to the class informing them about the survey. Online delivery has an extra step of entering a date period for the evaluation to be active in Cecil.
    The option to add optional questions will be checked by default.
    Click on the Nextbutton.
  1. Name the Evaluation, e.g. ‘Educ.101’ for a course, or Lecturer’s name. For online evaluations photo can be placed on the description area. Click on the Nextbutton.


  1. (Online delivery only) Specify the start and end date/time for the evaluation period. Click on the Nextbutton.
  1. (Optional) Add extra questionsup to a maximum of 18 questions in total. To add questions click on the question in the bottom box and click “Add”. This will add the question to the upper box. Reverse the process to remove a question.
    Questions will appear on the evaluation form in the order that you add them here. You can change the orders of the additional questions added by clicking on Move up or Move down links to the right of the upper box.
    If you don’t wish to add any additional questions simply skip this step by clicking on the Nextbutton.

  1. (Optional – Only if you had selected to publish an announcement to the class) Edit class announcement. There are some default texts with instructions for online delivery available or you can type in your own message. You can also select the option to email the announcement to the class as well. Click on the Nextbutton.

  1. Set permissions to people who will be allowed to view the report. Those granted permission will be able to view and print out reports from the ESC site once the survey has been processed. Click on the Nextbutton.
  1. Check the details of the evaluation, and confirm details. Click on the Create button.

  1. Your evaluation has been created. It has been given a unique evaluation number which will be used to create the evaluation report when the completed evaluation forms are scanned and uploaded.
    To print out the hard-copy form from PDF click on the Finish/Download button. When you print out the evaluation form, make sure FOUR BLACK BOXES ON EACH CORNER ARE CLEARLY PRINTED and not been cut off. This is crucial information for scanner, in order to process survey forms.
    Once the evaluation has been processed, you can view and reprint past reports by revisiting Student Evaluation website.
