A. HOTEL RESERVATION INSTRUCTION

1.  The form (page 2) is for U.S. weight category karate championships 2010 competitors, attendee and IKO members.

2.  Please use only one response method to make your reservation.

3.  H.I.S. TOURS U.S.A INC will handle your accommodations.

4.  You may submit your reservation request form via e-mail or facsimile as follows.

5.  Reservation form must be received by January 04, 2010 05:00PM PCT.

6.  Credit card payment only. Sorry, we cannot accept cash and check.

7.  We will send you an order confirmation via fax or E-mail when your request has completed.

B. CANCELLATION POLICY AND CHANGES TO EXISTING RESERVATIONS

1. Should a written notice of cancellation need to be faxed or E-mail as follows.

(a) One (1) night room charge will be applied, If cancelled after January 22, 2010.

(b) Full amount of room charges will be applied, if cancelled after January 26, 2010.

2. All changes that occur after a reservation has been confirmed needs to be fax or E-mail as follows but No later than January 26 2010 at 05:00PM PCT. We cannot accept any changes and cancellations after above date.

1.  This room rate includes ROOM and TAX only. All incidental charges must be paid own when you checked out.

2.  Reservations are processed on a first-com, first-served basis until January 04, 2010.

3.  Limited numbers of rooms are available for 3 nights from JAN 29 to Feb 01, 2010.

4.  After January 04, 2010 H.I.S.TOURS U.S.A. INC will not honor these rates and accommodation.
