Declaration & Consents

1. We understand that the acceptance of this application by the nursery is subject to the conditions stated in the nursery regulations and policy. We agree to accept those conditions and to be jointly and severally liable for the fees.

2. Should an emergency arise, we give consent for the nursery to admit my/our child to hospital and consent to emergency medical care as deemed necessary by a hospital medical practitioner.

3. We give consent for Calpol to be administered to our child should he/she suffer a fever at the following dosages up to 4 times in 24 hours: child 3-6 months 2.5 ml; child 6 to 24 months 5 ml; child 2 to 4 years 7.5 ml. NB. Parents must inform the nursery if this medication or the dosage is no longer suitable for the child. Yes/No (delete as appropriate)

regulations

Entry to the nursery

Application forms must be completed and returned before a child can be considered for entry into the nursery. On acceptance of a child to the nursery a non-refundable deposit of £100 will be required to secure the placement based on the agreed start date on the application form. The deposit will be deducted from the first invoice. If the start date is delayed on parent’s request for more than 2 weeks than that stated on the application form the nursery will forfeit the deposit and a further deposit will be required to secure a placement starting on a future date which will also be subject to availability.

General policy

The nursery operates in accordance with the “Policy” document which is available for all parents to read.

Hours of opening

Mondays to Fridays:Full day 7:30a.m. to 6.00 p.m.

Morning sessions including Breakfast & lunch 7.30 a.m. to 1:00 p.m.

Afternoon sessions including lunch & snacks 1:00 p.m. to 6:00 p.m.

Holidays:The nursery will be closed at Christmas, new year and all statutory bank holidays. No charges will be made when the nursery is closed during these holidays. Please refer to annual nursery calendar for the exact dates.

Absences

If your child is to be absent, the nursery should be notified by telephoning 01244 324422.

Sickness, Injury, & Medicine

In order to keep the spread of infection in the nursery to the minimum, we must ask that any child suffering from diarrhea, vomiting, doubtful rash, or any other infectious illnesses be kept away from the nursery.

If during the course of the day a child becomes ill or injured the nursery will contact the parents or nominated representative to ask them to pick up their child.

The nursery will administer medicine and apply cream if parents complete a medicine consent form. All medicine brought into the nursery must be clearly labeled and given to the staff.

Parents are required to check and sign the medicine administration or accident record books accordingly, as stipulated by Ofsted.

It is the parents’ responsibility to pick up any medicine left in the nursery for their child at the end of a session.

In the case of an emergency we should, of course, take a child to hospital and inform you at the same time.

Security

A child will not be allowed to leave the nursery with anyone unknown to the nursery staff unless previously arranged by the parents or guardian. If parents make a prior arrangement, the nursery will require the name of the person, a general description, and a password on arrival.

Clothing & Other Belongings

Every effort will be made to ensure children’s belonging is not damaged or lost. The nursery will not be held responsible for loss or damage to children’s belonging.

Children should be provided with suitable clothing for indoor and outdoor activities, especially in winter months. Additionally children need to bring with them each day a named bag containing the following items:

1. One flat sheet and one fitted sheet for babies. One flat sheet and one pillow case for toddlers.

2. Wellington boots (except babies).

3. A complete change of clothes, including underclothes.

4. Sun hat and sun cream for summer months.

5. Hat, gloves, and a warm coat for winter months.

All items need to be CLEARLY NAMED to avoid confusion.

Fees

The bookings given to us in the application form are regarded as the "normal bookings" and yearly invoices will be raised in accordance with this booking arrangement and then divided into equal monthly payments. Payment is due the first two weeks of the month. Payments not settled by the 4th week of the month will attract a late payment charge of £5. There is no reduction for any partly attended weeks. For children already started the nursery will require at least one month notice for any permanent changes to take effect. Any changes will be subject to availability. Notice to withdraw a child from the nursery must be given in writing at least one month in advance. Where such notice is not given a months fees must be paid in lieu of notice. Any additional charges incurred during the calendar month will be invoiced in arrears.

Late collection will be charged at £5 per every 15 minutes.

Half fees is allowed for 2 weeks per annum for family holidays. For a child joining the nursery on or after the first of July only 1 week of half fees will be allowed in the first year. Holiday have to be taken as a full week and cannot be taken as individual days. A holiday form must be completed and returned to the nursery before the first day of the month in which the holiday is taken. No charge is made for the Christmas and New Year closure and all Bank Holidays.

Data protection and information sharing

All information relating to the children in the nursery are treated as confidential. If the need arises for these information to be shared with any outside agencies the parents’ consent will be obtained. NB. Only under the extreme circumstances stated in our detailed policy will information be disclosed without parents’ consent.

Traffic and car parking on site

The nursery operates a one way traffic system for vehicles. There is a separate entrance and exit.

There are ample parking spaces on site and parents are discouraged to leave their vehicles on the road. Parents are reminded to exercise great care when driving on site and using the car park. Vehicles are left on site at the owner's risk. The nursery can accept no responsibility for injury, damage or loss to persons, vehicles or property.

General information

The nursery reserves the right to ask parents to remove a child if the principal feels that the nursery is not suitable for him/her. The same right applies if there is any breach of regulations.

Notice to withdraw a child from the nursery must be given in writing one month in advance. Where such notice is not given a months fees must be paid in lieu of notice.

Liability

The nursery accepts no liability for any pecuniary or other loss suffered by clients arising directly or indirectly as a result of the nursery being temporarily closed or the non-admittance of the children to the nursery for whatever reason.

Changes to regulations and policy

The nursery reserves the right to alter and amend these regulations at any time giving one month’s notice.
