Home health monitoring (also called telehomecare) is healthcare provided outside of a hospital setting. The technology used is proven, straightforward, efficient, secure, and confidential.
Once you have answered a series of questions, the telehomecareservice lets you send your personal health information from your home to a healthcare professional assigned to your case.
A healthcare professional consultsthis health information regularly in order to assessand monitor your health condition.
You can sign up to receive informational videos about your illness or health condition. The video contentsare based on healthcare and services best practices.
The telehomecare service also allows you to send questions to your healthcare professional and receive answers within a reasonable timeframe. While it is no substitute for emergency services, the health monitoring service is an easy way to communicate relevant information.
The device and how it works
The health monitoringservices can be accessed via an online app or a dedicated app installed on your computer, tablet, or smart phone (for the online app).As needed, a tablet (iPad) can be provided to you for a set period of time to be used exclusively to access the telehomecare.
Yourhealthcare professional or an appointed case worker will explain how to access the service from your device (desktop or mobile) orfrom a borrowed tablet.
What do we do with your data/information?
Your healthcare professional will analyze youranswers and determine the appropriate treatment for your health condition.
Based on your specific needs,your healthcare professional may contact other healthcare professionals. As needed, he/she will then call oremail you to adjust your care or treatment plan.
All of the information in your file will remain confidential.

From 8 a.m. to 4 p.m., Monday to Friday (except on statutory holidays).
Outside of operating hours, you can call Info-Santé at 811.
Telehomecareis not a replacement for emergency services. If medical attention is required, follow the instructions provided by your healthcare professional.
