Hollis Little League Manager and Coach Evaluation

Dear Parent,

Hollis LL requests a Manager and Coach Evaluation from each player’s parents. The League President reads each of the evaluations and uses them to help identify the most qualified managers and coaches for next season’s managers and coaches. Please take a few minutes to complete this evaluation form and return it in one of the ways described at the bottom. Your candid feedback is important. Managers and coaches are NOT given copies of their evaluations. Your individual responses will be kept strictly confidential. By submitting the Manager and Coach Evaluation, you are helping to ensure that we provide the best baseball instructors and role models for our children.
