HOB-1 Example Explanations

401a JSOn first round the patient was observed (PO) and was in compliance (Y)

On second round the patient was observed (PO) and was in compliance (Y)

Conclusion: Patient observed both times and in compliance both times. Count in N & D

401b AROn first round the patient was observed (PO) and was in compliance (Y)

On second round the patient was observed (PO) and was not in compliance (N)

Conclusion: Patient observed both times, not in compliance one of the times. Count in D;

do not count in N

402a FBOn first round the patient was observed (PO) and was not in compliance (N)

On the second round the patient was observed (PO) and was not in compliance (N)

Conclusion: Patient observed both times, not in compliance either time. Count in D;

do not count in N

402b MJOn first round the patient was not observed (N/A); also note N/A for compliance status

On second round the patient was observed (PO) and was in compliance (Y)

Conclusion: Patient only available for one observation and was in compliance.

Count in N & D

403a AMOn first round the patient was not observed (N/A); also note N/A for compliance status

On the second round the patient was observed (PO) and was not in compliance (N)

Conclusion: Patient only available for one observation and was not in compliance.

Count in D; do not count in N

403b GHOn first round the patient was not observed (N/A); also note N/A for compliance status

On second round the patient was not observed (N/A); also note N/A for compliance status

Conclusion: Patient not available for either observation. Do not count in N or D

404a MMOn first round it was noted that the patient was excluded (less than 18 yr of age or documented

contraindication) (E/C); also note E/C for compliance status

On second round it was noted that the patient was excluded (under 18 yr of age or documented

contraindication) (E/C); also note E/C for compliance status

Conclusion: Patient has a contraindication or exclusion for both observations. Do not

count in N or D

404b RTOn first round the patient was observed (PO) and was in compliance (Y)

On second round there was a documented contraindication (E/C); also note E/C for compliance

status

Conclusion: Patient had a contraindication on one observation but was without

contraindication or exclusion on the other observation, therefore is counted in the D;

patient was in compliance for the one observation that was not excluded, therefore count in

the N

405a SPOn first round the patient was observed (PO) and was not in compliance (N)

On second round the patient was a documented contraindication (E/C); also note E/C for

compliance status

Conclusion: Patient had a contraindication on one observation but was without

contraindication or exclusion on the other observation, therefore is counted in the D;

patient was not in compliance for the one observation that was not excluded, therefore do

not count in N

405b CCOn first round the patient was observed (PO) and was in compliance (Y). On the second round, the patient has expired, or is no longer on the ventilator, or has been transferred and is therefore not available for observation on the second round.

Conclusion: Because the patient was available for one observation without contraindication or

exclusion, it will be counted in the denominator; patient was in compliance and therefore it will be

counted in the numerator. If the patient had not been in compliance, it would not be counted in the

numerator.

405b DROn first round the patient has not yet been admitted to the unit, or is in the ICU and not on a ventilator.

However, the patient is in the ICU and on a ventilator without contraindication or exclusion for the

second observation (PO); the patient is not in compliance (N)

Conclusion: Count in the denominator because the patient was available for one

observation without contraindication or exclusion; patient was not in compliance, so it will

not be counted in the numerator. If the patient had been in compliance, it would have been

counted in the numerator.
