High Impact Families Offer

Families identified as meeting two or more of the HIF criteria come into the programme.

Where there is any uncertainty about whether the referred families meet the criteria and therefore whether they can be included in the Programme, a triage process takes place.

The triage currently involves representatives from Early Help, High Impact Families (hereafter known as HIF), Social Care, Education, Health, Children Centres and the Disabled Children’s Team. The triage will determine which pathway meets the needs of the family.

The triage can offer support help and signposting as well as assistance onto the HIF programme.

For example, where a family only hit one criteria or a Co-ordinator cannot be identified they will go to Early Help with help or advice. Where there is a Child Protection issue they will go to Social Care.

A case which is on the HIF programme will only gain access to the HIF offer if it has; Co-ordinator;Consent;a whole family assessment; aFOP (HIF tracking tool).

The Case Coordinator of each case under the HIF way of working has access to the following resources (subject to availability).

The Case Coordinator will need to submit a referral form (see back page) to organise access to these services.

Family Engagement Workers

  • The HIF programme currently employs a number of Family Engagement Workers. They will provide an interventionwhere existing agencies are unable to help.
  • The interventions will be specific, short term, and will complement the Families Plan.
  • Family Engagement Workers can support a new Co-ordinator completing a whole family assessment, help them to use and record on the Early Help System or update a FOP.

Employment

  • The HIF programme has access to a full time Employment Advisor, seconded from the Department of Work & Pensions.
  • The Employment Advisor will seek to engage with any member of any family included in the HIF programme, who is not in full time employment.
  • The HIF programme works closely with ‘Team North Somerset’, an employment/training/voluntary work provision aimed at hard to reach target groups.

Volunteer Family Coaches

  • Our coaches come from a range of partner agencies, including the local authority, as well as members of local community, and have a range of skills, experience and backgrounds. Our coaches are trained to work with a family or an individual, providing interventions to complement the work of partner agencies.
  • They empower families or individuals to make positive changes in their lives, enabling them to identify appropriate strategies and encouraging them to succeed in achieving their goals.

Youth Inclusion Support Project

  • 3 month interventions which reduce aim to reduce the risk of criminal behaviour and/or promote emotional and mental health wellbeing.
  • Intervention approach can be with individuals, groups or with families.

Young Person’s Substance Advice Service

  • Provides bespoke interventions related to risk/need in relation to young people at risk of or using drugs, alcohol or tobacco substances.

1 in 4 Telephone Counselling Service.

  • 6 - 12 sessions of free confidential telephone counselling.
  • Clients who may have had difficulties in accessing traditional face to face counselling may now receive the benefits of counselling support over the phone.
  • Fill out the referral form and email , your referral will then be assigned a qualified counsellor.
