Medicare Part D Options
Effective January 1, 2006
We are writing this note to Rossmoor patients who are enrolled through Affinity Medical Group in a Medicare Advantage health plan––either Health Net Seniority Plus or PacifiCare Secure Horizons.
A number of our patients are confused about the new Medicare Part D Prescription Drug plans for 2006. We want to make sure you make an informed Part D decision, without unintentionally losing your existing Medicare HMO benefits––
• Both Health Net Seniority Plus & PacifiCare Secure Horizons HMO plans will include Part D benefits in 2006.
• If you are a current Seniority Plus or Secure Horizons member, you will be automatically enrolled in their Medicare Part D plan beginning January 1, 2006.
• To avoid a lapse in medical coverage, YOU SHOULD NOT ENROLL in a separate Part D plan unless you intend to leave your current HMO plan.
- If you have already enrolled in a separate Part D plan, you will be automatically terminated by Medicare from your current health plan on January 1, 2006 and you will lose your HMO benefits.
Please call your health plan directly if you have questions, or you need to re-enroll for January 1.

PacifiCare Secure Horizons 800-228-2144Effective August 1, these Rossmoor physicians will be changing the name of their practice to Rossmoor Medical Associates and moving to a new office at 1210 Rossmoor Parkway (between Rossmoor Medical Center and the Bank of America building). They will continue to refer patientsYou will still have access to specialty care at Rossmoor Medical Center next door and specialists in theJohn Muir Medical Center Rossmoor Medical Center.
To keep your health insurance and your Rossmoor doctors:

To have your medical records transferred to the new location:
If you have not already done so, you must complete the enclosed form and return it to
the address printed on the form. The records will be copied and delivered to the new office
at no charge to you.
To schedule an appointment with your Rossmoor doctor:
Call (925) 939-1220 if calling on or before Thursday, July 28
Call (925) 933-1210 if calling on or after Friday, July 29.
