Directions: In this section you learned about the stages of change, the decision making model, and goal setting. Using the information you learned in the lessons, choose a health habit you would like to change and outline how you will make those changes.

This is the beginning of a semester-long assignment! You should work on this health habit/goal throughout the semester. In each section’s Reflection assignment, you will track your progress on the goal. For your final assignment in the course, you will evaluate your success.

Part I: Choose a Habit

After reviewing last week’s assignment, choose a health habit/goal that you would be willing to work on throughout the semester. The health habit can be from any of the dimensions of wellness (physical, emotional, intellectual, spiritual, social, or environmental).

Use the health habit you listed above to answer the questions in Parts II-IV.

Part III: Decision Making Model
