CASEWORKER

Work Area:
CarerServices
Based at:Leighton House, Rednal. Working across Birmingham and Solihullhospitals and in the community as required.
Required to work at any HBS site as requested.
North and South of Birmingham / Hours: Contracted 35 hours per week, Across 7 Days per week as required,including some evening/weekends.
Flexibility according to client/ job need but will need to work flexibly to cover hours and other days as required.

CORE OBJECTIVES OF POST
To provide an assessment of the family and the brain injury needs and then follow up these requirements accordingly, helping the family and BIP to move forward with their lives again.
KEY RESPONSIBILITIES:
  1. Effectively self manage workload/hours according to job requirements.
  2. Ensure all handwritten reports are accurate, and in clear, legible handwriting.
  3. The job will require you to meet specific targets as required by the funders and this will involve completing spreadsheets / monitors / surveys etc accurately and by the required times
  4. You will be required to produce reports, case studies, evaluations and outcomes that are well written and by the required deadlines
  5. Ensure that information about Headway’s services is communicated effectively to the relatives/carers of brain-injured people, being treated at local hospitals or otherwise becoming known to us.
  6. Spread such information more widely through talks, displays and distribution of literature to health and social services, Neighbourhood Offices, public libraries, CABs, the media and other
carers’ bodies and general community groups.
  1. Work closely with HBS Hospital Link Worker to ensure that people are reached at the point of discharge and where there are multiple needs.
  2. Liaise with hospital staff and with Social Services over hospital discharge and subsequent care plans
  3. Complete assessment of need and set agreed targets with service users. Offer support and encourage enablement so users can access services and get support, advice etc. in the relevant areas (Brain injury, support back to employment, financially, Housing, counselling).
  4. Produce all required personal monitoring items accurately and on time – e.g, end of month, expenses, outputs and timesheets etc.
  5. Deliver talks and displays to local organisations / network as required
  6. Produce items for the HBS newsletter e.g. Useful family contacts, updates and matters of general interest.
  7. Assist with publicity, special events and other occasions arranged by HBS
  8. To cover the enquiry line one day per week, answering calls from professionals, clients and carers requiring information about Headway services and brain injury
  9. Be part of the overall HBSstaff team, working flexibly and upholding the name of both organisations.
  10. Be flexible to suit the needs of the post and work fairly with the team as required, including takingturns in small extra duties outside of normal hours.
  11. Attend fully any training, coaching or team meetings etc as required, including outside the normal working day
  12. To be part of the Headway team, contributing to wider activities by volunteering on at least one outside of hours event and assisting on one special fund raising event .ect
  13. Uphold all Terms and Conditions of the post including covering in emergency situations as
required and upholding all other policies such as Health & Safety, Equal Opportunities, etc

EDUCATION & QUALIFICATIONS / GCSE/O-levels in English and Mathematics
Good writing skills and able to use good grammar & spelling / Experience of computer programs for WP/DTP/E-mail
NVQ II Advice and guidance
HEALTH & PRESENTATION / A neat and tidy appearance to gain immediate rapport with new contacts.
Able to cope with the demands of regular travel and varied hours.
Own car, insured for business use.
Domestic circumstances capable of supporting this type of employment. / In robust health, not subject to frequent illness.
KNOWLEDGE & RELEVANT
EXPERIENCE / Good appreciation of the local framework of health and social services provision.
Experience in dealing with people in traumatic/emotional situations.
Assessing
Knowledge of local services / Understanding of the consequences of brain injury
Experience of working with similar client groups.
Familiar with the benefits & services available to disabled persons and carers.
