SUMMARY

The Target Store at Hampton Village in St. Louis, Missouri, was reviewed and inspected by Thomas Branham, of Elevator Consulting Services Inc., on April 25th, 2006. My QEI number is 1339 and am certified by NAESA. NAESA is the National Association of Elevator Safety Authorities. We have identified corrective items that in our opinion need to be addressed by the Elevator Contractor and the General Contractor. They are free to offer an explanation or dispute any individual item within the report but we can offer a code, specification or reasonable reason for each and every negative evaluation. They are evaluated on and for the ASME 17.2 Code for Inspectors Manual and industry standards for acceptable installation and maintenance procedures.

PASSENGER ELEVATOR

The balance of the initial list from turnover was completed. The valve was now sealed and tagged. The unit still has leveling problems particularly in the down direction. The drip pan was not under the pump unit and a pipe support has to be removed in order to get in place. The hoistways, pits and car tops need to be cleaned. The performance times were satisfactory.

CART RETRIEVAL ELEVATOR

The door operation has the new closed loop control that was specified and is having a few issues but is operating well. The doors are smooth, do not slam and even look nice due to the stainless steel on the outer skin.

ESCALATORS

These are KONE ECO 3000 units. They both had a communication error at the time of the inspection that needed a reset. There was no indication what caused this other than a potential power outage. The units ran fine, were quiet and very clean.

The units have the new chains that supposedly do not need lubrication. One set of chains is already making barking noises and showing signs of rust or high friction. When first proposed by KONE, we agreed to have signage in both baskets warning about lubrication. Therefore, I am still asking for the additional signage for future protection as service techs change and may have relatively short memories.

The loaded gap test report was still missing from the down unit and needs to be onsite. The demarcation lighting for each respective unit was put on a toggle switch so it is on all the time. This should be controlled by a running circuit so they are only on when in operation. One bulb was already burned out or not operational.

CARTVEYOR

The unit was running well at the time of the inspection but apparently been problematic.

CORRECTIVE ITEMS

The following minimum items need corrective action taken. Documentation of completion is required.

  1. Provide an oil monitoring sheet on the controller and make regular entries per code. A17.1.8.6
  1. Provide MSDS sheets on the hydraulic oil and place in a plastic sleeve on the front of the controllers.
  1. Provide the controller data plate with the Prevailing Code Version identification.
  1. Install the fire emergency box in the elevator lobby. Provide keys that are properly tagged.
  1. The front side slow speed car doors are bottomed on the transom. Adjust the door arm relation to provide space.
  1. The rear side slow speed car doors are bottomed on the transom. Adjust the door arm relation to provide space.
  1. Adjust the front door operator chain. Too much slack.
  1. Adjust the rear door operator chain. Too much slack.
  1. Adjust the slow speed car door upthrusts on both front and rear openings. Need to be tighter.
  1. Lube the roll pin of each spring closure device (8).

CORRECTIVE ITEMS

The following minimum items need corrective action taken. Documentation on completion is required.

  1. Provide MSDS sheets for the hydro oil in the machine room.
  1. Provide the controller data plate with the Prevailing Code Version identified per code.
  1. Adjust the Wye Delta transfer. Should be no greater than 3 seconds.
  1. Clean the heavy rust off the rails.
  1. Clean the pit.

ESCALATORS 3/DN & 4/UP

CORRECTIVE ITEMS

The following minimum items need corrective action taken. Documentation on completion is required.

ESCALATOR THREE (DN)

  1. Provide the loaded gap test results for review. Unit was installed under the A17.1 2000 code per the data plate, code requires the test. Did not find the sheet in the upper basket, the up unit did have one.
  1. Adjust the upper left curve skirt clearance to be within code. Currently ¼ inch.
  1. Provide and install the “Hold Handrail” and other safety signage on both sides of the escalator, both top and bottom landings.
  1. Add signage to both baskets “No lube on step chains”. Can be a label printed off computer if necessary.
  1. The demarcation and skirt lighting should be on a running circuit. Currently on all the time even when not running. Modify the circuitry as needed to allow lighting only when running.

ESCALATOR FOUR (UP)

1.Provide the loaded gap test results for review. Unit was installed under the A17.1 2000 code per the data plate, code requires the test. No Brushes

2.Provide and install the “Hold Handrail” and other safety signage on both sides of the escalator, both top and bottom landings.

3.Add signage to both baskets “No lube on step chains”. Can be a label printed off computer if necessary.

4.The demarcation and skirt lighting should be on a running circuit. Currently on all the time even when not running. Modify the circuitry as needed to allow lighting only when running.

5.The inside of the step chain links are scraping the top side of the drive sprocket. There is high roller wear or dust on the chain. Adjust as needed to eliminate the squeaking from the top basket. If not the chain, find out the source and eliminate.

1.The bottom right side access area had a lot of long plastic shavings. The chains appear adjusted so they may be old but monitor, if additional long shavings accumulate adjust chain and guide accordingly.
