HAMILTON HIGH SCHOOL SPORTS RELEASE FORM

  1. Training Rule Policy: I acknowledge that I have received, read and understand the Hamilton High School Training Rule Policy, which any student participating in extracurricular activities must abide by.
  1. Injury Awareness: I am aware that there is a risk of being injured that is inherent as a result of participation in any high school sport or activity. I am aware that the injury may be severe, including, but not limited to fractures, brain injuries, paralysis, neck or spinal injuries, damage to all internal organs, serious injury to ligaments, muscles, tendons, and other aspects of the muscular skeletal systems or death, but nevertheless want my child to participate in high school activities.
  1. On behalf of my child, and myself I agree to indemnify and hold harmless Hamilton High School and its owners, operators, instructors, employees, agents, and servants from any and all claims, demands, costs, expenses, and compensation arising out of or in the course of or in any way related to any personal injury to my child or me.
  1. Parent Permission to Travel: I give my approval for my child’s participation in any of the organized and supervised activity programs conducted by Hamilton high under the following conditions:

*If chosen for a team or group which is to make an out-of-town trip, my child is to travel under the coach’s/sponsor’s direction and authority from the time of departure until the return to Hamilton. The coach, sponsor, school staff, or Board of Trustees will not be held liable or responsible in case of accident incurred during practices, contests or trips supervised by Hamilton Public Schools. All students are expected to ride to all school activities on district provided transportation. Students will not be allowed to participate if they arrive at a contest in transportation other than that provided by the school district. Exceptions may be made only in an emergency through prior approval of the Activities Director and/or Principal. Students may request to ride home after events with their parents and/or guardians. The final decision whether students ride home with parents/guardians lies with the head coach. Students in violation of transportation rules, involving alcohol, drugs, tobacco, theft, and curfew will be disciplined as follows:

a. Parent contacted and the student turned over to local authorities.

b. The student becomes the responsibility of the parent.

c. The parent will provide transportation home from the event.

d. The student will be disciplined as outlined by the Student Handbook.

  1. Medical Treatment: In consideration of permission granted my child or ward to participate in activities, I hereby authorize the sponsor or extra-curricular activities my child/ward is participating in at Hamilton High School. In case of injury to said child, to consent to any x-ray examination, anesthetic, medical or surgical diagnosis or treatment, and hospital care to be rendered to the minor under the general or special supervision and advice of any physician or surgeon licensed to practice in the State of Montana. It is understood that I will be notified of the injury at the earliest possible opportunity, but this authorization will allow treatment until I arrive. I also understand that the expenses incurred from any such treatment will be my responsibility.
