House Rules

  1. These rules and policies apply to all employees of «insert name of Pharmacy» including those on fixed term or period contracts, casual or part time employees and probationary employees.
  2. These house rules and policies may be amended, expanded or discontinued upon reasonable notice. Any changes will be discussed and the views of staff will be taken into account before any changes are implemented.

Security

  1. The Pharmacy has a strict security policy which must be followed at all times. Your assistance in maintaining the security of the Pharmacy is required.
  2. This Pharmacy has an Information Security Policy. Every employee and contractor must sign a Confidentiality Agreement recording their acceptance of their obligations under this policy.

Confidentiality

  1. You must keep all health and commercial information confidential. Health information must only be discussed with the staff member directly responsible for providing care to the patient concerned.
  2. If you have any concerns about confidentiality these must be raised with the Pharmacy Security Officer.

Serious Misconduct

  1. Serious misconduct is behaviour of a kind that cannot be tolerated at any time within a pharmacy, or offences that transgress law or regulations. Examples include:
  2. Unauthorised possession of company property. Staff must meticulously follow the staff purchases procedure to avoid possible misunderstandings.
  3. Falsification of company records to enable theft of cash or goods.
  4. The unauthorised access and/ordisclosure of confidential information or customers' personal information.
  5. Deliberate acts adversely affecting hygiene, safety or quality including the consumption of alcohol and illegal drugs.
  6. In each case it will be a question of determining the extent of the breach and the circumstances in which it occurred before deciding whether the matter constitutes a serious misconduct.
  7. We will carefully investigate any allegation of serious misconduct. This will include discussions with all persons who may be able to assist us in ascertaining the full situation. All factual matters ascertained will be put to you. You will have an opportunity to express your views on such matters and to provide any explanation.After a full and fair hearing has been accorded, you will be advised in writing of the decisionand the reasons for it.
  8. The following actions may then be taken:
  9. If there has been no misconduct no action will be required.
  10. If there has been misconduct that does not constitute serious misconduct a warning will be given to you.
  11. If there has been serious misconduct you will be dismissed. In this circumstance you will be paid up to the date of dismissal.

2Employment Contract amendments for pharmacies using the Pharmacy Guild employment contract

  1. Confidential Information
  2. The Health Information Privacy Code applies in the pharmacy. All information arising from the operation of the pharmacy, whether relating to the business or customers is confidential.
  3. The employee will not during, or at any time after employment, disclose to any unauthorized person any confidential information acquired during the course of their employment with the employer. This restriction will not apply to any disclosure authorized by the employer or required by law.
  4. The employee agrees that after the termination of employment, they will not retain the means for reproducing such information and will sign a declaration confirming the above if required.
  5. Every employee and contractor must sign a Confidentiality Agreement recording their acceptance of their obligations under this policy.

Confidential Information

The employee shall not during the currency of this Agreement or after its termination for whatever reason use, disclose or distribute to any person or entity, other than as necessary for the proper performance of their duties and responsibilities under this agreement., or as required by law, any confidential information, messages, data or trade secrets acquired by the Employee in the course of performing their services under this agreement. This includes but is not limited to information about the employer’s business.

The Employer and the Employee shall comply with the obligations set out in the Privacy Act 1993. The Employee must not breach the privacy of any customer or client in the course of their employment.

The Employee will be asked to agree to the Employer’s Security Policy. Agreement to this Policy is mandatory for all employees and contractors..
