1. Any club seeking re-affiliation and who is un-financial with the GDSFA relating to unpaid monies for fees, fines, penalties etc from earlier years will have their affiliated rejected until all outstanding monies are paid to the GDSFA.

2. Incomplete affiliation forms (all required paperwork and documents must be attached) will also be rejected until such time as the required documentation and information is provided.

3. Completed Affiliation Form is to be returned to GDSFA Office by 1st December 2015.

1. Clubs are required to list Club Sponsors who shall have any type of advertising appearing on the playing kit. (Note, advertising by cigarette companies is forbidden)

12. Terms of Application

1. The Club acknowledges and agrees that:

1. The Club applies for affiliation and will abide by GDSFA’s Constitution, Regulations, Policies and By-Laws;

2. The affiliation fee of $400.00 must be included with this application or placed in GDSFA Bank Account prior to submitting the Affiliation Form;

3. Current audited financial statement tabled at your 2015 Club AGM is attached to this application, ANNEX 3;

4. Copy of Club Constitution and By-Laws if changed is attached to this application, ANNEX 4;

Note: Clubs are required to attach with this page photos of their playing kit (or design graphics from the manufacturer). Clubs are to supply the Association with a soft copy of the image in JPEG or PDF format.

Annex 2: Club Logo

Note: Clubs are required to attach with this page an image / graphics of their Club logo. Clubs are to supply the Association with a soft copy of the image in JPEG or PDF format.

Annex 3: Financial Analysis

Clubs are required to provide with this Application Form.

1. Provide a copy of the Club’s Audited Profit and Loss Statement & Balance Sheet for the Financial Year ending 2015 submitted to your AGM.
